血清白蛋白水平与脑梗死复发关系的临床研究

摘　　要：目的研究血清白蛋白水平与脑梗死复发之间的关系。方法采用横断面病例对照研究。选取北京大学第一医院2004年5月至2015年5月收治的复发性脑梗死患者404例作为脑梗死复发组,并选取同期住院的非复发性脑梗死患者1 214例作为对照组。对比2组临床资料及血清白蛋白水平。结果复发性脑梗死组血清白蛋白明显低于对照组(P<0.01)。将血清白蛋白与体质量指数、血红蛋白、血脂进行Logistic回归分析,发现2组之间体质量指数、血红蛋白、血脂差异均无统计学意义(P>0.05),而复发性脑梗死组血清白蛋白仍明显低于对照组(P<0.01)。将血清白蛋白分为低白蛋白组和正常白蛋白组,并与年龄、性别及高血压病、糖尿病、高脂血症、冠心病、吸烟、饮酒史进行Logistic回归分析,低白蛋白血症、高龄、高血压病史在复发性脑梗死组更为显著(P<0.01)。结论复发性脑梗死患者的血清白蛋白较对照组偏低,这一结果与患者营养状况无明显关系,而低白蛋白血症可能是脑梗死复发的危险因素。Objective To study the relationship between serum albumin level and recurrence of ischemic stroke.Methods This was a cross-sectional case-control study. Four hundred and four cases of recurrent ischemic stroke treated in the first hospital of Peking University from May 2004 to May 2015 were selected as the recurrence group of ischemic stroke, and 1 214 cases of non recurrent ischemic stroke in the same period were selected as control group. The clinical data and serum albumin level were compared between the two groups.Results Levels of serum albumin in recurrent ischemic stroke group were significantly lower than those of control group （ P 〈0.01）. Logistic regression analysis showed no difference in body mass index, hemoglobin level and blood lipids level between two groups （ P 〉0.05）, whereas levels of serum albumin in recurrent ischemic stroke group was significant lower than those of control group （ P 〈0.01）. All the cases were divided into low serum albumin group and normal serum albumin group, and serum albumin together with age, sex and history of hypertension, diabetes mellitus, hyperlipidemia, coronary heart disease, smoking and drinking were compared. Recurrent ischemic stroke group showed lower serum albumin, higher percentage of advanced age and hypertension （ P 〈0.01）.Conclusion The serum albumin of patients with recurrent ischemic stroke was lower than that of the control group, and there was no significant relationship between the results and the nutritional status of the patients. Low serum albumin may be a risk factor for recurrence of ischemic stroke.
