白内障超声乳化联合小梁切除术治疗难治性青光眼合并白内障的疗效观察

摘　　要：目的探讨白内障超声乳化联合小梁切除术治疗难治性青光眼合并白内障的疗效。方法根据连续入组的方法选择2015年11月~2016年12月在本院入院治疗的难治性青光眼合并白内障患者100例(100眼),按入组序列号奇偶数进行平均分组,观察组50例(50眼)采用超声乳化联合小梁切除术治疗,对照组50例(50眼)采用白内障囊外摘除联合小梁切除术治疗,记录两组患者术前与术后各时间点(1周、1个月、3个月)视力、眼压、散光度及并发症。结果 (1)观察组术后各时间点裸眼视力、矫正视力改善均明显优于对照组,差异具有统计学意义(均P<0.05);(2)两组患者术后各时间点眼压比较差异不明显,差异无统计学意义(均P>0.05);(3)观察组术后各时间点散光度均明显低于对照组,差异具有统计学意义(均P<0.05);(4)与对照组术后发生并发症发生率(16%)相比较观察组明显较低(6%),差异具有统计学意义(P<0.05)。结论白内障超声乳化联合小梁切除术治疗难治性青光眼合并白内障可有效降低眼压、改善视力效果良好,且安全性高,值得临床推广应用。Objective To discuss the curative effect of phacoemulsification combined with trabeculectomy in the treatment of refractory glaucoma with cataract. Methods One hundred cases（100 eyes） of refractory glaucoma with cataract patients in our hosipital form November 2015 to December 2016 were selected according to the method of continuous entry group, and were divided into the observation group and the control group according to the odd even number of the sequence number, 50 cases （50 eyes） in each group. The observation group were treated with phacoemulsification combined with trabeculectomy, the control group were treated with extracapsular cataract extraction combined with trabeculectomy. The visual acuity, intraocular pressure, astigmatism and complication were recorded before operation and each time point after the operation（1 week,1 month,3 months）. Results （1）The improvement of uncorrected visual acuity and corrected visual acuity of the observation group at each time point after operation were obviously better than those of control group, which were statistically significant（all P 〈0.05）. （2）The intraocular pressure of the two groups at each time point after operation was no obvious difference, which was statistically significant（all P 〉0.05）. （3）The astigmatism of the observation group at each time point after operation was obviously lower than that of control group, which was statistically significant（all P〈0.05）.（4）The postoperative complications incidence of the observation group was 3（6%）, and significantly lower than 8（16%） of the control group, which was statistically significant（P 〈0.05）. Conclusion Cataract phacoemulsification combined with trabeculectomy in the treatment of refractory glaucoma with cataract can effectively reduce the eye pressure of the fundus and improve the effect of vision, and high safety, which is worth to be spread.
