胰腺节段切除术对胰腺良性肿瘤患者的疗效及并发症分析

摘　　要：目的分析胰腺节段切除术治疗胰腺良性肿瘤的临床疗效与并发症发生情况。方法 2012年1月—2017年1月,于黑龙江省中日友好医院学习期间收治的胰腺良性肿瘤患者中随机选取100例,以手术方式为依据,分为两组,对照组患者进行传统手术切除,观察组患者采用胰腺节段切除术进行治疗,对比两组临床治疗效果与并发症发生情况。结果观察组患者平均手术时间、平均住院时间分别为(202.02±32.08)min、(12.28±2.29)d,短于对照组的(289.56±45.28)min、(20.56±4.28)d,平均术中出血量为(190.22±20.28)m L,少于对照组的(239.38±35.68)m L,(t=20.487 5、23.1585、19.980 0,P<0.05);观察组50例患者中,3例胰瘘,1例腹腔出血,1例腹腔感染,并发症发生率为10.00%;对照组50例患者中,1例胰瘘,8例腹腔出血,8例腹腔感染,并发症发生率为34.00%,观察组患者并发症发生率相较于对照组的更低(χ2=6.258 9,P<0.05)。结论胰腺节段切除术治疗胰腺良性肿瘤的临床疗效显著,有利于降低手术并发症发生率,安全性较高,存在良好的临床推广价值。Objective This paper tries to analyze the clinical efficacy and complications of pancreatectomy in the treatment of benign pancreatic tumors. Methods From January 2012 to January 2017, 100 patients with pancreatic benign tumor treated during the study period in China-Japan Friendship Hospital of Heilongjiang Province were selected and divided into two groups according to the surgical method. The patients in the control group underwent conventional surgical resection.Patients in the test group were treated with resection of the pancreatic segment and the clinical efficacy and complications of the two groups were compared. Results The average operative time and average length of stay in the observation group were（202.02±32.08）min and（12.28±2.29）days, respectively, shorter than those in the control group（289.56±45.28） min and（20.56±4.28）days. It was（190.22±20.28）m L, which was less than the（239.38±35.68）m L of the control group（t=20.487 5,23.158 5, 19.980 0,P〈0.05）; among the 50 patients in the observation group, 3 were pancreatic fistulas, and 1 was abdominal hemorrhage. 1 In the case of abdominal infection, the complication rate was 10.00%; in the control group of 50 cases, 1 case of pancreatic fistula, 8 cases of abdominal bleeding, and 8 cases of abdominal infection, the complication rate was34.00%. The incidence of complications was lower in the observation group than in the control group（χ2=6.258 9, P〈0.05）.Conclusion Pancreatic segmental resection in the treatment of benign pancreatic tumors has a significant clinical effect,which is beneficial to reduce the incidence of surgical complications, high safety, and has a good clinical promotion value.
