摘　　要：目的研究Claudin-4在胃癌组织中的表达,分析其与临床病理的关系及其意义。方法选取2015年3月至2016年9月在西安市中心医院普通外科行手术切除并经病理证实的胃癌标本50例,癌旁正常组织28例,癌前病变的标本23例。采用免疫组化检测胃癌、癌前病变、癌旁正常组织中Claudin-4的表达情况,分析其与胃癌临床病理特征的关系。结果胃癌组织、癌前病变组织、癌旁正常组织中Claudin-4蛋白的阳性率分别为54.00%、52.17%、39.28%,随着病变进展Claudin-4呈逐渐增高趋势,Claudin-4蛋白胃癌组织、癌前病变组织、癌旁正常组织中的阳性率比较,差异无统计学意义(P>0.05)。不同年龄、性别、TNM分期及淋巴结转移情况胃癌患者的胃癌组织标本Claudin-4蛋白阳性率比较,差异无统计学意义(P>0.05);不同分化程度胃癌组织标本Claudin-4蛋白阳性率比较,差异具有统计学意义(P<0.05)。结论 Claudin-4蛋白表达与胃癌分化程度密切相关,在高分化腺癌中表达阳性率明显高于低分化腺癌。Objective To detect the expression of Claudin-4 in gastric cancer tissue, and analyze its relationship withclinicopathology and its significance. Methods Fifty cases of gastric cancer specimen from March 2015 to September 2016 atgeneral surgery department in the Xi＇an Central Hospital, 28 cases normal tissues specimen and 23 cases of pre-cancerouslesion specimen were collected in the study. The expression of Claudin-4 in gastric cancer tissue, pre-cancerous lesion tissueand normal tissues were detected by immunohistochemical detection, and its relationship with clinicopathology characteristicswere analyzed. Results The positive expression rates of Claudin-4 in gastric cancer tissue, pre-cancerous lesion tissue andnormal tissues were 54.00%, 52.17% and 39.28% respectively. The expression of Claudin -4 increased gradually with theprogression of lesions. There were no significant differences in positive expression rates of Claudin-4 among gastric cancertissue, pre-cancerous lesion tissue and normal tissues（P〉0.05）. There were no significant differences in positive expression ratesof Claudin-4 in gastric cancer patients with different age, sex, TNM period and lymph node metastasis（P〉0.05）. There wassignificant difference in positive expression rate of Claudin-4 in gastric cancer patients with different differentiation （P〈0.05）.Conclusion Expression of Claudin-4 protein in gastric cancer is closely related to differentiation. The positive rate of Claudin-4in high-differentiated adenocarcinoma is significantly higher than low-differentiated adenocarcinoma.
