摘  要:目的探讨细胞增殖核抗原Ki-67与环氧合酶2(COX-2)在结直肠癌组织中的表达水平及意义。方法选取我院收治的120例结直肠癌患者为研究对象,采用免疫组织化学法检测患者结直肠癌组织及癌旁组织中的Ki-67与COX-2的阳性表达率,分析Ki-67与COX-2与结直肠癌临床病理特征和预后的关系及结直肠癌组织中Ki-67和COX-2表达的相关性。结果结直肠癌组织、存在淋巴结转移、TNM分期Ⅲ期+Ⅳ期患者中的Ki-67及COX-2阳性表达率显著高于结直肠癌旁组织、不存在淋巴转移、TNM分期Ⅰ期+Ⅱ期的患者(P<0.05)。结直肠癌组织中Ki-67和COX-2表达呈正相关(r=0.571,P<0.05)。Ki-67阳性组和阴性组的5年生存率分别为62.07%、76.36%,COX-2阳性组和阴性组的5年生存率分别为53.26%、68.29%,差异具有统计学意义(P<0.05)。结论 Ki-67与COX-2在结直肠癌组织中表达增高,其阳性表达参与肿瘤形成、发展及转移,且表达水平与患者不良预后相关。Objective To investigate the expression levels of nuclcar-associated antigen Ki-67 and cyclooxygenase-2(COX-2) in colorectal carcinoma and their significance. Methods A total of 120 colorectal carcinoma patients treated inour hospital were selected as the study objects. The positive expression rates of Ki-67 and COX-2 in colorectal carcinomatissue and carcinoma adjacent tissues in all the patients were detected by immunohistochemical method. The relationshipbetween Ki-67, COX-2 and clinicopathological features, prognosis of colorectal carcinoma and the correlation between Ki-67 and COX-2 expression in colorectal carcinoma were analyzed. Results The positive expression rates of Ki-67 and COX-2in colorectal carcinoma tissues and patients with lymph node metastasis, TNM stage Ⅲ+Ⅳ were higher than those incolorectal carcinoma adjacent tissues and patients without lymph node metastasis, TNM stage Ⅰ+Ⅱ(P〈0.05). Positivecorrelation showed between Ki-67 and COX-2 expression in colorectal carcinoma (r=0.571, P〈0.05). The 5 year survivalrates of Ki-67 positive group and negative group were 62.07%, 76.36% respectively, and that of COX-2 positive group andnegative group were 53.26%, 68.29% respectively, the differences were statistically significant (P〈0.05). Conclusion Theexpression of Ki-67 and COX-2 increase in colorectal cancer tissues, and their positive expression are involved in tumorformation, development and metastasis, and the expression levels are related to the poor prognosis of the patients.
