外周血循环肿瘤细胞检测在前列腺癌患者预后判断中的应用探讨

摘　　要：目的探讨循环肿瘤细胞(CTCs)在前列腺癌患者预后判断中的应用价值。方法本回顾性研究共纳入2015年1月至2017年6月在首都医科大学附属北京友谊医院泌尿外科住院治疗的68例前列腺癌患者及50例健康体检对照者。收集前列腺癌患者(研究组)及对照组外周血3.2 ml,利用Cyttel-CTC-imFish检测系统检测CTCs数目。根据CTCs检测结果,将前列腺癌患者分为CTCs阳性组(35例)和CTCs阴性组(33例)。同时记录患者年龄、血常规、生化指标、出凝血时间、前列腺特异性抗原(PSA)、前列腺核磁共振扫描、CT及骨扫描等结果。统计学分析CTCs数目与上述指标的相关性。结果研究组患者的CTCs阳性率明显高于对照组,差异具有统计学意义(P<0.05)。CTCs阳性组患者的PSA数值明显高于CTCs阴性组,差异具有统计学意义(P<0.05)。发生转移的患者CTCs阳性率明显高于未发生转移患者,差异具有统计学意义(P<0.05)。CTCs阳性组患者血红蛋白(HGB)水平明显低于CTCs阴性组,而血小板(PLT)、纤维蛋白原(FIB)、D-二聚体(D-D)、碱性磷酸酶(AKP)、乳酸脱氢酶(LDH)水平明显高于CTCs阴性组,两组之间差异均具有统计学意义(P<0.05)。结论前列腺癌患者CTCs水平与肿瘤发生、肿瘤转移、相关血清指标存在相关性,对预测患者的疾病预后有辅助作用。Objective To analyze the value of circulating tumor cells（ CTCs） monitoring in the prognosis of patients with prostate cancer.Methods There were totally 68 prostate cancer patients which were treated in Beijing Friendship Hospital and 50 control patients which underwent physical examination recruited in this study. The research time was during January 2015 and June 2017. 3. 2 ml peripheral blood of prostate cancer（ research group） and control group were collected from in-patients. Cyttel-CTC-im Fish detecting system was used to detect the CTCs. The patients with prostate cancer were divided into CTCs positive group（ n = 35） and negative group（ n = 33） according to the positive results of CTCs. Age,blood routine,biochemical routine,blood clotting routine,prostate-specific antigen（ PSA）,MR,CT and bone scan were collected simultaneously. The correlation between the number of CTCs and the above results were analyzed. Results The CTCs positive ratio in control group was significantly lower than that of the research group（ P〈0. 05）. The value of PSA in CTCs positive group was significantly higher than that of the CTCs negative group（ P〈0. 05）. The CTCs positive ratio in patients with tumor metastasis was significantly higher than that of the non-metastasis group（ P〈0. 05）. The value of hemoglobin（ HGB） in CTCs positive group was significantly lower than that of the CTCs negative group（ P〈0. 05）. The values of platelet（ PLT）,alkaline phosphatase（ AKP）,lactate dehydrogenase（ LDH）,fibrinogen（ FIB） and D-Dimer（ D-D） in CTCs positive group were higher than those of the CTCs negative group（ P〈0. 05）. Conclusion The level of CTCs in patients with prostate cancer is associated with tumor metastasis,and has relevance with the levels of HGB,PLT,AKP,LDH,FIB and D-D,which can be used to evaluate the disease prognosis.
