摘 要:目的探讨X线、CT及MRI对骨巨细胞瘤(giant cell tumor,GCT)硬化边征象检出及其病理基础探讨。方法回顾性分析不同影像检查方法及不同部位骨巨细胞瘤硬化边的有无、形态及发生几率。结果 72例GCT病例中,X线所示硬化边征象的比率最低,为17.5%,CT次之为50%,MRI所示病变边缘各序列低信号环的比率最高为82.6%。不同部位硬化边几率不同,CT所示脊椎最高为63.7%。结论 CT所示GCT边缘部分线样硬化边及MR所示低信号环为骨巨细胞瘤的常见征象,可能与病灶对周围骨质压迫引起反应性硬化以及力线的重构有关。MR所示低信号环几率高于CT所示硬化边,可能与病变边缘具有纤维薄膜有关。Objective To rediscover the sclerotic margin of giant cell tumor and to explore the pathomechanism. Methods X-ray, CT and MRI of 72 cases of giant cell tumor confirmed by pathology were analyzed retrospectively. The probability of occurrence and existence of the sclerotic margin of giant cell tumor were summarized. Results Of 72 the cases, the sclerotic margin probability of X-ray was the lowest (17.5%), while, CT and MRI were 50% and 82.6%, respectively. The probability of sclerotic margin had varied a lot with different locations. The sclerotic margin probability of spine was the highest with CT. Conclusion Part of sclerotic margin is the common sign of giant cell tumor. The mechanism of sclerosis are related to oppression of surrounding structure with the tumor, reconstruction of power line and the fibrous capsule.
