摘　　要：目的探讨Nd:YAG激光与超声洁刮治联合治疗重度慢性牙周炎的临床价值。方法选取2015年9月至2016年9月本院收治的重度慢性牙周炎患者96例,共201颗牙,均为第一磨牙,随机分为实验组和对照组,实验组48例患者共99颗,对照组48例患者共102颗。实验组采用Nd:YAG激光与超声洁刮治联合治疗,对照组采用超声洁刮治治疗。观察并比较两组治疗前后的龈沟出血指数(sulcus bleeding index,SBI)、探诊深度(probing depth,PD)和附着丧失(attachment loss,AL)等临床指标。结果两组患者治疗后SBI、PD及AL较治疗前均有所改善(P<0.05),但实验组改善情况优于对照组,差异有统计学意义(P<0.05)。结论 Nd:YAG激光联合超声洁刮治是治疗慢性牙周炎的有效策略,能有效提高患者生活质量,值得临床推广。Objective To observe the clinical effect of Nd：YAG laser combined with ultrasonic subgingval scaling and root planning SRP verus SRP alone in treatment of severe chronic periodontitis. Methods 96 patients with severe chronic periodontitis who were referred to our hospital from September 2015 to September 2016 were randomly assigned to the test group Nd：YAG laser combined with SRP or the control group SRP alone. The sulcus bleeding index（SBI）, probing depth（PD）, and attachment loss（AL）were respectively recorded and analyzed at baseline and repeated after treatment. Results Significant improvement existed in SBI, PD and AL after treatment in both groups（P〈0.05）.The improvement in observation group was more obvious compared to control group（P〈 0.05）. Conclusion Nd：YAG laser combined with ultrasonic subgingval scaling and root planning is a reliable,effective treatment for patients with severe chronic periodontitis. It can involve the quality of life and worthy of being widely applied in clinic.
