新疆喀什地区应用羊膜治疗严重眼表病的临床研究

摘　　要：目的分析在新疆喀什地区应用羊膜治疗严重眼表病的临床效果。方法收集2017年1月至12月因常见致病微生物引起的角膜溃疡住院手术治疗的50例（50眼）为治疗组，2016年3月至9月因相同致病微生物引起的角膜溃疡住院药物治疗的50例（50眼）为对照组。对比两组患者并发症发生率、有效率、复发率、住院天数和住院费用。结果治疗组和对照组：并发症发生率分别是8．00％和36．00％，差异有统计学意义（P=0．001）、有效率分别是86．00％和66．00％，差异有统计学意义（P=0．001）、复发率分别是14．00％和34．00％，差异有统计学意义（P=0．019），治疗组复发率降低，不再多次住院，因此总住院费用较对照组低。结论羊膜治疗是药物治疗严重眼表病的又一选择，特别是在眼表病多发而角膜移植手术又严重滞后的新疆喀什地区。Objective To analyze the clinical efficacy of amniotic membrane for the treatment of severe ocular surface disease in Kashgar region of Xinjiang. Methods The treatment group,50 eyes of 50 cases with corneal ulcer caused by pathogenic microorganism from Jan. 2017 to Dec. 2017 received surgery. The control group,50 eyes of 50 cases with corneal ulcer caused by the same pathogenic microorganism from Mar. 2016 to Sep. 2016 received drug treatment. The complications rate, the total effective rate, recurrence rate, average length of stay and the average hospitalization expenses were compared between two groups. Results The total complication rates in treatment group and control group were 8.00% and 36.00%, respectively. The difference was statistically significant （P =0.001 ）. The total effective rate were 86.00% and 66.00%, respectively. The difference was statistically significant （P = 0. 001 ）. The recurrence rates were 14.00% and 34.00%, respectively, the difference was statisticaliy significant （P = 0. 019 ）. In treatment group the recurrence rate reduced, without many times in the hospital, the total hospital cost was reduced. Conclusion Amniotic membrane is another option in the treatment of severe ocular surface diseases, especially in Kashgar region, Xinjiang. The ocular surface diseases often occur and corneal transplantation backward in Kashgar region, Xinjiang.
