摘  要:目的:检测急性髓细胞白血病(AML)患者骨髓标本中DNA甲基转移酶3A(DNMT3A)亚型1、2的表达情况,分析其临床意义。方法:应用SYBR Green real-time PCR方法检测DNMT3A1、DNMT3A2在成人初诊原发AML患者和非血液病患者(对照组)骨髓有核细胞中的表达情况,并分析其与AML分型、年龄、性别、白细胞计数、危险分层等临床参数的关系。结果:DNMT3A1及DNMT3A2在AML患者中表达水平较对照组无明显差异。但亚型分析显示,DNMT3A2在伴有RUNX1-RUNXT1、伴有NPM1突变及伴有CEBPa双突变的AML患者中,表达较对照组显著升高(均P<0.05);但在非特指(NOS-M5)患者中,表达较对照组显著下降(P<0.05)。DNMT3A2及DNMT3A2与DNMT3A1比值,在非急性早幼粒细胞白血病的低危组患者中显著升高(P<0.01)。DNMT3A2表达水平与血小板及巨核细胞数呈显著负相关(均P<0.05)。结论:DNMT3A2在部分AML患者中,尤其是低危患者中表达显著升高,而在非特指型急性单核细胞白血病中表达显著下降,这种差异性的表达在AML发病及治疗选择中可能有一定的价值。Objective:To detect the expressions of DNA methyltransferase 3 Aisoform 1 and 2 in bone marrow cells of adult patients with de novo acute myeloid leukemia(AML)and analyze its clinical significance.Method:The expression of DNMT3 A1 and DNMT3 A2 in adult patients with newly diagnosed de novo AML or non-hematological diseases(as the control group)was detected by SYBR Green real-time PCR method,and we analyzed its relationships with clinical data such as AML subtype,age,sex,white blood cell count,and risk stratification.Result:There was no significant difference of DNMT3 A1 and DNMT3 A2 detected between patients with AML and control group.Subtype analysis showed that DNMT3 A2 was significantly increased in AML patients with RUNX1-RUNXT1,NPM1 mutation and CEBPa double mutation(all P〈0.05),while decreased in AML not otherwise specified(NOS)with maturation(P〈0.05).DNMT3 A2 and the ratio of DNMT3 A2 to DNMT3 A1 significantly increased in low risk group of non-APL patients.The expression of DNMT3 A2 was negatively correlated with platelet count and megakaryocyte count(both P〈0.05).Conclusion:DNMT3 A2 is significantly increased in partial AML patients,especially the low risk group of non-APL patients,but decreased in AML NOS with maturation.This differential expression indicates that alteration of DNMT3 Aisoforms expression might play roles in the pathogenesis of AML,which might be a potential biomarker for AML prognosis or treatment.
