摘 要:目的探讨CT、MRI对颅底沟通性脑膜瘤的诊断价值,旨在提高对特殊类型脑膜瘤的认识和诊断水平。方法回顾性分析经手术病理证实的28例颅底沟通性脑膜瘤患者的CT、MRI影像学资料,比较CT和MRI检查判定结果与术后病理检查结果的一致性。结果颅底沟通性脑膜瘤在MRI上T1WI多表现为等信号,T2WI多表现为稍高信号,在CT上多表现为等密度,增强多呈明显强化。CT与MRI检查单独诊断颅底沟通性脑膜瘤符合率的比较差异无统计学意义(P>0.05)。CT对颅底沟通性脑膜瘤侵犯周围骨质的诊断准确率高于MRI(P<0.05)。MRI对颅底沟通性脑膜瘤侵犯周围软组织的诊断准确率高于CT(P<0.05)。CT及MRI联合诊断与病理结果具有良好的一致性(K=0.764)。结论 CT及MRI在颅底沟通性脑膜瘤的诊断中各有优势,两者联合诊断可明显提高诊断,可为临床治疗及手术方式的选择提供重要依据。Objective To explore the diagnostic value of CT and MRI in the diagnosis of communicating meningiomas of the skull base and improve the diagnosis of special type meningiomas. Methods The CT and MRI imaging data of 28 patients with cranial base communicating meningiomas confirmed by surgery and pathology were retrospectively analyzed. The consistency between CT and MRI were compared with the results of postoperative pathological examination. Results On the MRI,the T1 WI of the cranial base meningiomaappeared as the equal signal,and the T2 WI was a little high signal,and the same density was shown on the CT,and the enhancement was obviously enhanced. There was no significant difference in coincidence rate between CT and MRI in diagnosing skull base communicating meningiomas(P〉0.05). The diagnostic accuracy of CT for skull base communicating meningioma invading the surrounding bone was higher than that of MRI(P〈 0.05).The diagnostic accuracy of MRI for skull base communicating meningioma invading surrounding soft tissue was higher than that of CT(P〈0.05). The combined diagnosis of CT and MRI was in good agreement with pathological findings(K=0.764). Conclusion CT and MRI have advantages in the diagnosis of communicating skull meningiomas.Combined diagnosis can significantly improve the diagnostic efficiency,so as to provide an important basis for clinical treatment and operation mode selection.
