放疗同步替吉奥+顺铂治疗局部晚期非小细胞肺癌的临床研究

摘　　要：目的评价放疗同步替吉奥(S-1)+顺铂治疗局部晚期非小细胞肺癌(LA-非小细胞肺癌)的疗效及安全性。方法 24例局部晚期非小细胞肺癌患者,随机分为A组和B组,各12例。所有患者均行三维适形放疗(3D-CRT),A组采用放疗同步替吉奥+顺铂治疗,B组采用放疗同步多西他赛+顺铂治疗。比较两组近期疗效、无进展生存期(PFS)、总生存期(OS)及毒副反应。结果 A组近期总缓解率(ORR)为75.0%,近期疾病控制率(DCR)为83.3%,B组近期ORR和DCR均为100.00%;两组近期ORR和DCR比较差异均无统计学意义(P>0.05)。A组和B组的中位PFS分别为8.1个月和11.6个月,比较差异有统计学意义(P<0.05);A组和B中位OS分别为17.9个月和22.7个月,比较差异无统计学意义(P>0.05)。A组Ⅲ~Ⅳ度脱发发生率低于B组,差异有统计学意义(P<0.05);两组白细胞减少、中性粒细胞减少、血小板减少、粒细胞减少性发热、恶心、呕吐、疲乏、放射性食管炎、神经毒性发生率比较差异均无统计学意义(P>0.05)。结论放疗同步替吉奥+顺铂治疗局部晚期非小细胞肺癌中位PFS劣于标准同步放化疗方案。Objective To evaluate the efficacy and safety of radiotherapy combined with S-1 plus Cisplatin for the treatment of locally advanced non-small cell lung cancer.Methods 24 patients with locally advanced non-small cell lung cancer were randomly divided into group A and group B,12 cases in each group.All patients were treated with three dimensional conformal radiotherapy（3 D-CRT）.Group A was given radiotherapy combined with S-1 plus Cisplatin for treatment,group B was given radiotherapy combined with Docetaxel plus Cisplatin for treatment.The recent efficacy,progression free survival（PFS）,overall survival（OS） and side effects between the two grous were compared.Results The recent overall remission rate（ORR） and recent disease control rate（DCR） of group A were 75.0% and 83.3% respectively,those of group B were all 100.00%,there were no statistically significant differences in recent ORR and recent DCR between the two groups（P〈0. 05）.The median PFS of group A and group B were 8.1 months and 11.6 months respectively,the difference was statistically significant（P〈0. 05）.The median OS of group A and group B were 17.9 months and 22.7 months respectively,there was no statistically significant difference（P〈0. 05）.The incidence of Ⅲ-Ⅳ degree alopecia in group A was significantly lower than that in group B,the difference was statistically significant（P〈0. 05）.There were no statistically significant differences in incidence of leukopenia,neutropenia,thrombocytopenia,febrile neutropenia,nausea,vomiting,fatigue,radiation esophagitis and neurotoxicity between the two groups（P〈0. 05）.Conclusion Radiotherapy combined with S-1 plus Cisplatin for the treatment of locally advanced non-small cell lung cancer is inferior to the standard concurrent chemoradiotherapy regimen in PFS.
