连朴饮联合四联疗法治疗消化性溃疡(湿热阻胃)随机平行对照研究

摘　　要：[目的]观察连朴饮联合四联疗法治疗消化性溃疡(湿热阻胃)疗效。[方法]使用随机平行对照方法,将130例门诊患者按就诊顺序号随机分为两组。对照组65例四联疗法:阿莫西林,1g/次,2次/d;甲硝唑,400mg/次,3次/d;奥美拉唑40mg/次,2次/d;枸橼酸铋钾,220mg/次,2次/d。治疗组65例连朴饮(半夏制、川黄连、石菖蒲各9g,厚朴制、香豉炒各12g,芦根15g);疼痛强烈加蒲黄;腹胀加枳壳、柴胡;反酸加瓦楞子;恶心加半夏、竹茹;热重加蒲公英、黄芩;湿气重加苍术、白扁豆,1剂/d,水煎400m L,2次/d;四联疗法同对照组。连续治疗14d为1疗程。观测临床症状、幽门螺杆菌、不良反应。治疗1疗程(14d),判定疗效。随访1年,观测复发率。[结果]治疗组临床痊愈30例,显效17例,有效14例,无效4例,总有效率93.84%;对照组临床痊愈24例,显效17例,有效13例,无效11例,总有效率86.07%;治疗组疗效优于对照组(P<0.05)。幽门螺杆菌清除率治疗组优于对照组(P<0.05)。随访1年,治疗组复发复发率25.00%,对照组复发率45.83%,治疗组疗效优于对照组(P<0.05)。[结论]连朴饮联合四联疗法治疗消化性溃疡(湿热阻胃),疗效满意,无严重不良反应,值得推广。[Objective] To observe the curative effect of Lianpu Yin combined with quadruple therapy on peptic ulcer（Shire Zuwei）. [Methods] Using random parallel control method, 130 outpatients were randomly divided into two groups according to the order number. The control group had 65 quadruple therapy： amoxicillin, 1 g/times, 2 times/d; metronidazole, 400 mg/times, 3 time/d; omeprazole 40 mg/times, 2 times/d; bismuth potassium citrate, 220 mg/times, 2/d. In the treatment group, there were 65 cases of Lianpu Yin（Zhibanxia, Chuanhuanglian, Shicangpu each 9 g, Zhihoupo, Chaoxiangchi each 12 g, Lugen 15 g）, severe pain plus Puhuang, abdominal distentionpluw Zhiqiao, Chaihu, anti acid plus Walengzi, nausea plus Banxia, Zhuru, ardent fever plus Pugongying, Huangqin, eczema plus Cangzhu, Baibiandou, 1 doses/d, decoction 400 m L, 2/d The triple therapy was the same as the control group. The continuous treatment of 14 d was 1 courses. Clinical symptoms, Helicobacter pylori and adverse reactions were observed. The 1 course of treatment（14 d） was treated and the curative effect was determined. After 1 years of follow-up. [Results] 31 cases were cured in the treatment group, 17 cases were markedly effective, 14 cases were effective, 4 cases were invalid and the total effective rate was 93.84%. The control group cured 24 cases, 17 cases, 11 effective, 11 cases, total effective rate 86.07%, the curative effect of the treatment group was superior to the control group（P〈0.05）. The clearance rate of Helicobacter pylori in the treatment group was better than that in the control group（P〈0.05）. Follow up for 1 years, the recurrence rate in the treatment group was 25%, and the recurrence rate in the control group was 45.83%. The treatment group was better than the control group（P〈0.05）. [Conclusion] The curative effect of Lianpu Yin combined with triple therapy on peptic ulcer（Shire Zuwei） is satisfactory, and there is no serious adverse reaction. It is worth promoting.
