25G玻璃体切除术联合内界膜剥除术并曲安奈德球内注射治疗特发性黄斑前膜的临床效果观察

摘　　要：目的探讨25G玻璃体切除术联合内界膜剥除术并曲安奈德球内注射治疗特发性黄斑前膜的临床疗效。方法回顾性分析医院收治的38例(共40眼)特发性黄斑前膜患者的临床资料。其中术中接受曲安奈德球内注射治疗设为观察组,未予曲安奈德球内注射治疗的为对照组,每组各19例,20眼。记录、比较2组术后1周,1个月、3个月、6个月最佳矫正视力(BCVA)与黄斑中心凹厚度值(CMT)结果。结果 2组患者术后BCVA较术前明显提高,CMT较术前明显减少(P<0.01);术后1个月,观察组BCVA较对照组改善明显(P<0.01);术后1周,观察组CMT较对照组下降明显(P<0.05)。结论内界膜剥除术联合曲安奈德球内注射治疗特发性黄斑前膜能够快速减轻黄斑水肿,利于视力早期恢复与改善,值得临床推广应用。Objective To explore the clinical effect of 25 G vitrectomy combined with internal limiting membrane peeling and triamcinolone acetonide injection in the treatment of idiopathic macular epiretinal membrane. Methods The clinical data of 38 patients（ 40 eyes） with idiopathic macular epiretinal membrane were retrospectively analyzed. Patients who received intraoperative injection of triamcinolone acetonide were selected as the observation group and the rest patients as control group,with 19 cases in each group（ 20 eyes）. The BCVA and CMT were recorded and compared between the 2 groups at 1 weeks,1 months,3 months,and 6 months after operation. Results The BCVA of the 2 groups increased significantly compared with that before operation,and CMT was significantly decreased compared with that before operation（ P〈0. 01）. After1 months,the BCVA in the observation group improved more significantly than that in the control group（ P〈0. 01）. After 1 weeks,the CMT of the observation group decreased more significantly than that of the control group（ P〈0. 05）. Conclusion25 G vitrectomy combined with internal limiting membrane peeling and triamcinolone acetonide injection in the treatment of idiopathic macular epiretinal membrane can reduce macular edema quickly and improve the recovery and improvement of early postoperative visual acuity. It is worthy of clinical application.
