摘　　要：目的:通过对比腹腔镜和开腹手术行原发灶切除联合D_2淋巴结清扫治疗老年进展期胃癌的效果,为老年进展期胃癌的临床治疗提供有价值的参考依据。方法:选择收治的108例老年进展期胃癌患者为研究对象,根据手术方法的不同分为腹腔镜组和开腹组,对比两组的治疗效果。结果:与开腹组相比,腹腔镜组出血量明显减少,术后恢复流食时间、胃管留置时间以及住院时间缩短,差异均具有统计学意义(P<0.05)。结论:腹腔镜下行原发灶切除联合D_2淋巴结清扫术有助于提高老年进展期胃癌的治疗效果。Objective To compare the efficacy of laparoscopic and open radical resection combined with D2 lymph node dissection in the treatment of elderly patients with advanced gastric cancer,so as to provide valuable reference for the clinical treatment of elderly patients with advanced gastric cancer. Method 108 elderly patients with advanced gastric cancer who were admitted to our hospital were selected as research subjects. They were divided into endoscopic group and open group according to different operative methods. The therapeutic effects of two groups were compared. Results Compared with laparotomy group,laparoscopic group can significantly reduce the amount of bleeding,shorten the postoperative recovery time,liquid gastric tube indwelling time and hospitalization time,the difference was statistically significant（ P〈0.05）. Conclusion Laparoscopic resection of primary foci combined with D2 lymph node dissection can help to improve the therapeutic effect of advanced gastric cancer in the elderly.
