摘  要:目的:探讨多层螺旋CT诊断早期急性阑尾炎的价值。方法:对81例早期急性阑尾炎患者进行多层螺旋CT诊断,并探讨其诊断价值。结果:多层螺旋CT确诊出80例急性阑尾炎患者,确诊率98.77%。经手术病理确诊81例急性阑尾炎,其中坏疽性阑尾炎14例(17.28%)、单纯性阑尾炎20例(24.69%)、蜂窝组织炎性阑尾炎21例(25.93%)、化脓性阑尾炎26例(32.10%);其中有9例伴阑尾四周脓肿(11.11%),39例伴阑尾粪石(48.15%),50例伴淋巴结肿大(61.73%)。比较两种方法诊断率差异无统计学意义(P>0.05)。多层螺旋CT诊断早期急性阑尾炎的特异度、敏感度、准确度高达90.12%、97.53%、98.77%。结论:为早期急性阑尾炎患者进行多层螺旋CT诊断,结果较为理想且安全可靠,值得临床普及应用。Objective To explore the value of multi-slice spiral CT in the diagnosis of early acute appendicitis. Method 81 cases of early acute appendicitis in our hospital were studied,and all patients were diagnosed with multi-slice spiral CT,and the diagnostic value was discussed. Results 80 cases of acute appendicitis were diagnosed by multi-slice spiral CT,accounting for98. 77%. By surgery pathology confirmed 81 cases of acute appendicitis,in which 14 cases of gangrenous appendicitis( 17. 28%),simple appendicitis 20 cases( 24. 69%),cellular tissue inflammatory 21 cases( 25. 93%),suppurative appendicitis appendicitis 26 cases( 32. 10%). Among them,9 cases were associated with periappendiceal abscess( 11. 11%),39 cases with appendectomy( 48. 15%),and 50 patients with lymphadenopathy( 61. 73%). The diagnostic rate of the two methods was not statistically significant( P〈0.05). The specificity,sensitivity and accuracy of muti-slice spiral CT in the diagnosis of early acute appendicitis were as high as 90. 12%,97. 53%,98. 77%. Conclusion Multi-slice spiral CT diagnosis for early acute appendicitis is ideal and safe. It is worthy of clinical popularization.
