摘 要:目的:探索强迫症患者的静息态脑功能网络特征。方法:纳入符合美国精神障碍诊断与统计手册第4版强迫症诊断标准的门诊患者62例,以及性别、年龄和受教育程度相匹配的正常对照36例。采用耶鲁-布朗强迫量表(Y-BOCS)、贝克抑郁量表(BDI-Ⅱ)、状态-特质焦虑问卷(STAI)评估强迫症患者强迫、抑郁和焦虑症状。所有被试进行静息态脑电波采集,分析5个频段[δ(1~4 Hz)、θ(4~8Hz)、α(8~15 Hz)、β(15~30 Hz)、γ(30~48 Hz)]的脑功能网络拓扑属性,其网络属性参数包括标准化的集群系数(C_(nml))、标准化的最短路径长度(L_(nml))。结果:在脑电波测查中,5个频段中标准化的集群系数(C_(nml))组间差异均无统计学意义(均P>0.05),当固定阈值cost=12%时,在δ、α、以及β频段,强迫症患者组的标准化的最短路径长度(L_(nml))小于正常对照组[δ:(1.4±0.2)vs.(1.5±0.2);α:(1.3±0.2)vs.(1.4±0.2);β:(1.4±0.2)vs.(1.5±0.2);均P<0.05]。在强迫症患者组中,所有5个频段条件下,其网络属性参数和与Y-BOCS总分和分量表分之间的相关系数和偏相关系数均无统计学意义(r=0.00~0.24)。结论:本研究显示强迫症患者的脑功能网络在δ、α和β频段可能存在"小世界"网络属性的异常。Objective: To examine the characteristic of resting-state brain functional network in patients with obsessive-compulsive disorder (OCD). Method: Sixty-two OCD out-patients meeting the criteria of the Diagnostic and Statistical Manual of Mental Disorders,Fourth Edition (DSM-IV) for OCD and 36 normal controls who matched gender,age and years of education participated in this study.The Yale-Brown Obsessive Compulsive Scale (Y-BOCS),Beck Depression Inventory (BDI-II) and State Trait Anxiety Inventory (STAI) was separately used to assess OC,depression and anxiety symptoms severity for the OCD patients.Electroencephalogram (EEG) was collected from all of subjects in resting-state to analyze separately the topological features of functional network at five bands [ δ (1-4 Hz), θ (4-8 Hz), α (8-15 Hz), β (15-30 Hz), γ (30--48 Hz)].The parameters of topological features included normalized cluster coefficients (C nml ) and normalized characteristic path length (L nml ). Results: The normalized cluster coefficients (C nml ) were not significantly different between the OCD and controls groups at any of EEG band ( Ps 〉0.05).The normalized characteristic path length (L nml ) values were lower in OCD patients than in the controls at δ , α ,and β bands while the cost was set in 12% [ δ :(1.4±0.2) vs.(1.5±0.2); α :(1.3±0.2) vs.(1.4±0.2); β :1.4±0.2 vs.1.5±0.2; Ps 〈0.05].These parameters were not significant correlated with Y-BOCS total and sub-scale scores ( r =0.00-0.24). Conclusion: It suggests that the altered small-world network (SWN) properties at δ , α ,and β bands might be correlated with the OCD.
