MRI与螺旋CT对直肠癌术前分期对照分析

摘　　要：目的:对比MRI与螺旋CT对直肠癌术前分期的诊断效果。方法:选择经病理检查确诊的直肠癌患者110例,对其进行MRI与螺旋CT检查。将病理结果作为金标准,对比螺旋CT与MRI对本组患者T分期与N分期的诊断效果。结果:MRI对本组T_1~T_2期诊断的准确率为93.33%,高于螺旋CT的50.00%,差异具有统计学意义(P<0.05);螺旋CT与MRI对本组T3期与T4期诊断准确率对比差异无统计学意义(P>0.05)。MRI对本组患者N分期诊断的灵敏度与特异度高于螺旋CT组,差异具有统计学意义(P<0.05);螺旋CT与MRI对本组患者N分期诊断的阴性预测值、阳性预测值与准确率对比差异无统计学意义(P>0.05)。结论:相较于螺旋CT,MRI在直肠癌T_1~T_2分期诊断及N分期灵敏度与特异度方面更具优势,可以作为首选方法应用于临床。Objective：To compare the diagnosis of preoperative staging of rectal cancer with MRI and spiral CT.Methods：110 cases of rectal cancer diagnosed by pathological examination were selected and examined by MRI and spiral CT.The pathological results were used as the gold standard to compare the diagnostic effects of spiral CT and MRI on T staging and N staging in this group.Results：The accuracy rate of MRI in diagnosis of T_1~T_2 in this group was 93.33%,higher than that in spiral CT 50%,the difference was statistically significant（P〈0.05）.There was no significant difference in the diagnostic accuracy of T_3 and T_4 between the two groups（P〈0.05）.The sensitivity of MRI for this group of patients with N staging and specificity was higher than that of spiral CT group,the difference was statistically significant（P 0.05）;negative predictive spiral CT and MRI in the group of patients with N staging diagnosis value,positive predictive value and accuracy comparison showed no significant difference（P〈0.05）.Conclusion：Compared with spiral CT,MRI has more advantages in the T_1~T_2 staging of rectal cancer and the sensitivity and specificity of N staging.It can be used as the first choice for clinical application.
