摘  要:目的检测hMLH1、ERCC1和Ki67在食管鳞状细胞癌及正常食管黏膜鳞状上皮的表达情况,研究错配修复系统对食管鳞癌发生发展的影响,探讨其成为食管鳞癌个体化治疗及临床预后判定指标的可行性。方法应用免疫组织化学技术检测120例食管鳞癌和20例正常食管鳞状上皮组织中hMLH1、ERCC1和Ki67的表达。结果 hMLH1和ERCC1在食管鳞癌组的阳性表达率均显著低于正常食管黏膜组(P<0.05);Ki67在食管鳞癌组的阳性表达率显著高于正常食管黏膜组(P<0.05)。食管鳞癌组中hMLH1、ERCC1与Ki67表达显著负相关,hMLH1与ERCC1表达不相关(P>0.05)。结论错配修复系统参与食管鳞状细胞癌的发生、发展过程,对评估食管鳞癌的进展情况并制定合理化疗方案具有重要指导意义。Objective To detect the expression of hMLH1,ERCC1 and Ki67 in esophageal squamous cell carcinoma and normal esophageal mucosal squamous epithelium,to study the effect of mismatch repair system on the development of esophageal squamous cell carcinoma,and to explore its application as an individualized therapy for esophageal squamous cell carcinoma and the feasibility of prognosis indicators.Methods Immunohistochemistry was used to detect the expression of hMLH1,ERCC1 and Ki67 in 120 cases of esophageal squamous cell carcinoma and 20 cases of normal esophageal squamous epithelium.Results The positive rates of hMLH1 and ERCC1 in esophageal squamous cell carcinoma were significantly lower than those in normal esophageal mucosa(P〈0.05).The positive rate of Ki67 in esophageal squamous cell carcinoma was significantly higher than that in normal esophageal mucosa(P〈0.05).In the esophageal squamous cell carcinoma group,the expression of hMLH1 and ERCC1 was significantly negatively correlated with the expression of Ki67,and the expression of hMLH1 was not correlated with the expression of ERCC1(P〉0.05).Conclusion The mismatch repair system is involved in the occurrence and development of esophageal squamous cell carcinoma.It has important guiding significance for evaluating the progress of esophageal squamous cell carcinoma and establishing a reasonable chemotherapy plan.
