摘 要:目的探讨趋化因子受体4(C-X-C chemokine receptor 4,CXCR4)在乳腺癌中的表达及其与乳腺癌诊断和预后的相关性。方法收集48例临床诊断并经病理确诊的原发性乳腺癌患者,采用逆转录实时荧光定量PCR(Reverse transcription-quantitative real-time PCR,RT-qPCR)的方法检测乳腺癌及邻近癌旁组织中CXCR4 mRNA的相对表达;收集48例乳腺癌和40例乳腺腺病患者的石蜡组织切片,采用免疫组织化学的方法检测乳腺组织中CXCR4蛋白的表达情况并分析其与临床病理指征的相关性。结果乳腺癌组织中CXCR4在mRNA和蛋白水平均高表达,显著高于癌旁组织和乳腺腺病患者(P<0.001);CXCR4蛋白对于乳腺癌诊断的敏感性和特异性分别达77.08%和80.0%;乳腺癌组织中CXCR4基因与乳腺癌患者的雌激素受体(ER)、孕激素受体(PR)、Ki-67增殖指数、分子分型密切相关。结论 CXCR4在乳腺癌组织中高表达且与有关预后指标密切相关,对于乳腺癌患者的诊断和预后评估具有重要的临床价值。Objective This study aims to investigate the expression of chemokine receptor 4( CXCR4) in breast cancer and its correlation with the diagnosis and prognosis of breast cancer.Methods Forty-eight cases of breast cancer and the adjacent para-cancerous tissue that came from the patients with primary breast cancer diagnosed by clinical diagnosis and confirmed by pathology were collected. To detect the relative expression of CXCR4 mRNA in breast cancer and the adjacent para-cancerous tissue,reverse transcription-quantitative real-time PCR( RT-qPCR) was used. For CXCR4 protein expression detection,48 cases of breast cancer paraffin sections and 40 cases of breast adenosis paraffin sections were collected and analyzed by immuno-histochemistry.Furtherly,the statistic analysis of the correlation between CXCR4 protein expression and clinicopathological indications was performed.Results Result showed that both mRNA and protein levels of CXCR4 in breast cancer tissues were significantly higher than those in the adjacent para-cancerous tissue and breast adenosis tissue( P〈0. 001).Further statistic analysis revealed that CXCR4 protein had a high sensitivity( 77.08%) and specificity( 80. 0%) for the diagnosis of breast cancer.Moreover,the CXCR4 gene expression in breast cancer tissues was closely related to ER,PR,Ki-67 proliferation index,molecular classification in breast cancer patients.Conclusion The high expression of CXCR4 in breast cancer is closely related to the prognostic indexes,which has important clinical value for breast cancer diagnosis and prognosis evaluation.
