腹腔镜宫颈癌根治术治疗早期宫颈癌的效果

摘　　要：目的探讨腹腔镜宫颈癌根治术(LRH)治疗早期宫颈癌的效果。方法选择2013年12月~2018年3月于我院就诊的早期宫颈癌患者120例,按手术方式不同分为研究组(60例)和对照组(60例)。研究组患者采用LRH,对照组患者采用开腹宫颈癌根治术(ORH)。比较两组患者的手术情况、术后恢复情况、并发症情况和癌症患者生命质量测定量表(FACT-G)评分。结果研究组患者的手术时间长于对照组,术中出血量少于对照组,清扫的淋巴结数目明显多于对照组(P=0.000);两组的切除阴道壁长度比较,差异无统计学意义(P=0.654)。研究组患者的术后排气、排便时间,留置导尿管和住院时间均明显短于对照组(P=0.000),术后并发症总发生率显著低于对照组(P=0.024)。两组术前的FACT-G评分比较,差异无统计学意义(P>0.05);与术前比较,两组患者术后6个月的FACT-G评分均明显高于术前(P=0.000),且研究组术后6个月的FACT-G评分明显高于对照组(P=0.000)。结论LRH治疗早期宫颈癌的效果较好,术后恢复快、并发症少,有助于提高患者术后的生活质量。Objective To laparoscopic radical hysterectomy（LRH） treating early-stage cervical cancer.Methods All of120 patients with early-stage cervical cancer treated in our hospital from December 2013 to March 2018 were selected and they were divided into the study group（60 cases） and control group（60 cases） according to the different surgical approach.Patients in the study group were treated with LRH and patients in the control group were treated with open radical hysterectomy（ORH）.The surgery efficacy,postoperative recovery,complications and FACT-G score of cancer patients were compared between the two groups.Results The operation time of the study group was longer than that of the control group,and the bleeding volume during operation was fewer than that of the control group,and the number of lymph nodes dissection in the study group was significantly more than that in the control group（P=0.000）.There was no significant difference in the length of excisional vaginal wall between the two groups（P=0.654）.The postoperative exhaust,defecation time,indwelling catheter and hospitalization time in the study group were significantly shorter than those in the control group（P=0.000）,and the total incidence of postoperative complications was significantly lower than that of the control group（P=0.024）.There was no significant difference in FACT-G scores between the two groups（P〉0.05）.Compared with before operation,FACT-G score of the two groups were significantly higher than those before operation（P=0.000）,and FACT-G score of the study group was significantly higher than that of the control group（P=0.000）.Conclusion LRH treating early-stage cervical cancer has a better effect with faster postoperative recovery and fewer complications,which is helpful to improve the quality of life of patients after operation.
