腹水脱落细胞DNA倍体对良恶性腹水的诊断价值研究

摘　　要：目的探讨腹水脱落细胞的DNA倍体分析对腹水良恶性质的诊断价值。方法采用Feulgen染色及图像分析仪技术(ICM)测定DNA的含量及倍体,以腹水脱落细胞学病理检查、胃肠镜下活检组织病理或手术组织病理发现肿瘤细胞或组织为金标准,对临床良性腹水54例、恶性腹水63例进行诊断价值分析。结果恶性疾病组腹水脱落细胞DNA倍体异性型阳性率为84.13%,明显高于良性疾病组腹水的14.81%;DNA倍体分析对癌性腹水诊断灵敏度为84.13%,特异度为85.19%,漏诊率为15.87%,误诊率为14.81%,阳性预测值为86.89%,阴性预测值为82.14%;与恶性腹水脱落细胞病理学检测相比,脱落细胞DNA倍体检测的灵敏度高,漏诊率低。结论腹水脱落细胞DNA倍体对腹水良恶性的诊断有一定价值,具有一定的临床推广价值。Objective To investigate the diagnostic significance of DNA ploidy of ascites exfoliated cells in the diagnosis of benign and malignant ascites. Methods 54 cases of benign ascites and 63 cases of malignant ascites were analyzed to determine the DNA content and ploidy analysis by Feulgen staining and ICM method. All cases were diagnostic criteria by cytological pathological examination of ascites exfoliative cytology, gastrointestinal histopathology or surgical pathology. Resultes Positive rate of DNA ploidy analysis in malignant ascites（84.13%）was significantly higher than in benign ascites（14.81%）. DNA ploidy analysis of malignant ascites sensitivity was84.13%, specificity was 85.19%, misdiagnosis rate was 15.87%, misdiagnosis rate was 14.81%, positive predictive value was 86.89%, and negative predictive value was 82.14%. Compared with malignant ascites exfoliative cytopathology, DNA ploidy detection has high sensitivity and low misdiagnosis rate. Conclusions Ascites exfoliated cells DNA ploidy in the diagnosis of benign and malignant ascites has a certain value, and provide guidance for the clinical.
