胰岛素抵抗与脑血管病的关系

摘　　要：目的 :探讨胰岛素抵抗与脑梗死、脑出血之间的关系。方法 :选取 34例脑梗死 ,4 4例脑出血病人 ,分别检测血压、血脂、空腹血糖 (FPG)、空腹胰岛素 (FINS)等各项指标 ,30例健康人作为对照组。结果 :脑梗死、脑出血组与正常对照组比较 :FPG、FINS明显增高 ,而胰岛素敏感指数 (ISI)明显降低 ;收缩压 (SBP)、舒张压 (DBP)、总胆固醇 (TC)、甘油三脂 (TG)、低密度脂蛋白 (LDL)、载脂蛋白B(apoB)也升高 ;相反 ,高密度脂蛋白 (HDL)、载脂蛋白A(apoA)、apoA/apoB降低。脑梗死与脑出血两组之间比较 ,脑出血组的DBP明显高于脑梗死组 ,其余指标均无统计学差别。结论 :胰岛素抵抗是脑血管病的独立危险因素 。Objective:To study the relationship between insulin resistance(IR),cerebral infarction (CI)and cerebral hemorrhage(CH).Methods:Blood pressure,blood lipid,fasting plasma glucose(FPG),fasting serum insulin(FINS), were measured in CI group (34 cases),CH group(44 cases)and 30 healthy control subjects.Results:In the CI group and CH group,FPG,FINS,systolic pressure(SBP),diastolic pressure(DBP),total cholesterol(TC),triglyceride(TG),low density lipoprotein(LDL),apolipoprotein B(apoB) were significantly higher than those in the control group,on the contrary,insulin sensitivity index (ISI),high density lipoprotein(HDL),apolipoprotein A(apo A),apoA/apoB were significantly lower than those in the control group.Compared CI group with CH group,DBP in CH group was higher than that in CI group.Coclusion:IR is the independent risk factor in cerebral vscular disease,but has no relationship with stroke type.
