轻度认知功能障碍患者血小板脂质筏内胆固醇水平的研究

摘　　要：目的研究轻度认知功能障碍(MCI)患者血小板脂质筏内胆固醇水平的变化,为其早期诊断提供科学依据。方法选择51例MCI患者、40例AD患者和52例认知功能正常的健康对照者,分别进行神经功能评定,采用以Optiprep为介质的密度梯度离心方法分离血小板脂质筏,应用点印迹的方法进行鉴定后,采用荧光法测定其内胆固醇水平。结果在离心管25%~30%Optiprep溶液界面处可见一条白色的闪亮环带,经点印迹法证实,其为脂质筏。MCI组和AD组血小板脂质筏内胆固醇水平均高于对照组,差异有统计学意义(P<0.01)。MCI组血小板脂质筏内胆固醇水平低于AD组,差异有统计学意义(P<0.05)。MCI组血小板脂质筏内胆固醇水平与MoCA总分及MoCA评分中的延迟回忆、注意力和计算力、视空间与执行功能亚项呈显著负相关(P<0.05)。AD组血小板脂质筏内胆固醇水平与MMSE评分中的回忆能力呈显著负相关(P<0.05)。结论血小板脂质筏内胆固醇水平的改变在MCI的诊断中有一定的潜在价值,有望成为MCI和AD早期诊断和病情评估的重要生物学标记物。Objective To investigate the changes of cholesterol of lipid raft in platelets in patients with mild cognitive impairment,and provide a scientific basis for its early diagnosis. Methods The neurological functions of 51 patients with MCI,40 patients with AD and 52 subjects with normal cognition were evaluated by neuropsychological examinations.Lipid raft was isolated by Optiprep gradient centrifugation and identified by dot blotting.The cholesterol level was measured by using fluorescence. Results A white light-scattering band under light illumination located at interface between 25%～30%.Optiprep was detectable,which was confirmed to be lipid raft by dot-blotting.The platelet lever of cholesterol in lipid raft with MCI group and AD group was statistically significantly higher than that with the control group （ P 〈0.01）,and the MCI group’s was lower than that in the AD group which was significantly different （ P 〈0.05）.In MCI group,the platelet lever of cholesterol in lipid raft was negatively correlated to the total points,delayed recall,attention and calculation,visuospatial and executive function of MoCA Scale.In AD group,the platelet lever of cholesterol in lipid raft was negatively correlated to memory capacity of Mini-Mental State Examination Scale. Conclusion The change of cholesterol level in platelet lipid raft has a certain potential value in the diagnosis of MCI and is expected to be an important biological marker for early diagnosis and evaluation of MCI and AD.
