帕洛诺司琼联合地塞米松在复发鼻咽癌放疗中顺铂增敏所致恶心呕吐的作用

摘　　要：目的研究帕洛诺司琼联合地塞米松在复发鼻咽癌放疗中顺铂增敏所致恶心呕吐的作用。方法选取我院收治的鼻咽癌复发患者93例,采用倾向性得分匹配法分成2组,顺铂化疗时只用帕洛诺司琼治疗组(对照组)和帕洛诺司联合地塞米松治疗组(研究组),观察不良反应、生活质量等。结果研究组头痛、头晕、疲劳明显降低(P<0.05);研究组止吐效果延迟期、全期优于对照组(P<0.05);研究组KPS评分、体质量变化较对照组号(P<0.05);延长住院时间,对照组为21%,研究组为6%,研究组明显好于对照组(P<0.05)。结论对复发鼻咽癌放疗中顺铂增敏所致恶心呕吐采用帕洛诺司琼联合地塞米松,能够减少帕洛诺司琼的药物不良反应,降低顺铂引起的恶心呕吐,提高生活质量,且不延长住院时间,安全性较好。Objective To investigate the effect of palonosetron combined with dexamethasone on nausea and vomiting induced by cisplatin sensitization in radiotherapy for recurrent nasopharyngeal carcinoma.Methods Ninety-hree recurrent nasopharyngeal carcinoma patients were divided into two groups by propensity score matching method.Control group were treated only by palonosetron treatment in the cisplatin chemotherapy,and the dexamethasone treatment group（study group）were by palonosetron combined with dexamethasone.We observed adverse reactions and quality of life.Results The headache,dizziness and fatigue of the study group were significantly decreased（P〈0.05）.The delayed period and the whole period of the antiemetic effect in the study group were better than that of the control group（P〈0.05）.The KPS score and body weight change in the study group were higher than those in the control group（P〈0.05）;prolonged hospital stay,21%in the control group,6% in the study group,the study group was significantly better than the control group（P〈0.05）.Conclusion Palonosetron combined with dexamethasone can reduce the side effects of palonosetron and reduce the nausea and vomiting caused by cisplatin in the treatment of nausea and vomiting caused by cisplatin sensitization in radiotherapy for recurrent nasopharyngeal carcinoma.It does not extend the hospital stay and has a better safety.
