摘  要:目的综合颈动脉超声多普勒成像、ABCD^2评分评价短暂性脑缺血(TIA)患者进展为缺血性脑梗死的风险。方法连续纳入50例TIA患者进行前瞻性研究,所有患者进行ABCD2评分并分组,在TIA发作后24h内进行颈部超声多普勒动脉成像。分别在TIA发作后1周、3个月、6个月对患者进行随访,将患者分为脑梗死组和非梗死组进行统计分析。结果 15例患者(30.0%)在随访中发生脑梗死,其中12例(80.0%)发生于3个月内。ABCD^2评分高危组更易发生脑梗死,同时脑梗死组颈动脉狭窄>50%者比例显著增高(p<0.05)。ABCD2评分和脑血管狭窄程度之间正相关(P=0.006)。结论综合彩色多普勒及ABCD^2评分能提高对TIA后缺血性脑卒中的预测能力。Objective Combining carotid ultrasound doppler imaging with ABCD2 score to evaluate the risk of transient ischemic attack( TIA) progressing to ischemic cerebral infarction. Methods 50 patients with TIA were continously chosen for prospectie study.All patients were divided into groups according to ABCD2 score and performed carotid ultrasound doppler imaging within 24 hours after onset of TIA.Patients were followed 1 week,3 months,6 months after TIA attack and divided into cerebral infarction group and non infarction group to analyze.Results 15 patients( 30.0%) in the follow-up visit occcured cerebral infarction,including 12 cases( 80.0%) occurred within 3 months.Cerebral infarction was much easier to occur in high risk group according to ABCD2 score and propotion of carotid stenosis over 50 percent in cerebral infarction group increased obviously( P〈0.05).There was a significant positive correlation between ABCD2 score and cerebrovascular stenosis( P = 0.006). Conclusion Combining color doppler with ABCD2 score can improve the prediction ability of ischemic stroke after TIA.
