广泛性焦虑量表-7在中国综合医院住院患者中的信效度研究

摘　　要：目的:评估中文版广泛性焦虑障碍量表(the generalized anxiety disorder 7-item scale,GAD-7)在国内综合医院住院患者中应用的信效度。方法:以简明国际神经精神访谈(MINI)作为"金标准",评价GAD-7中文版的效标效度。方便抽取四川大学华西医院肿瘤科、心脏内科、疼痛科、风湿免疫科及甲乳科569例住院患者进行GAD-7评测,在完成初次评定后的第10天采用系统抽样方法随机抽取其中40例患者进行GAD-7重测,并连续纳入不同分数段的150例患者于调查当天进行中文版MINI诊断性访谈。使用SPSS 18.0分析处理数据。结果:GAD-7的Cronbach's系数为0.90,各条目间的相关系数为0.483~0.712(P均<0.01),各条目与GAD-7总分的相关系数为0.734~0.820,重测信度系数为0.76(P<0.01)。在GAD-7分界值为9分时,灵敏度(66.70%)和特异度(85.80%)最佳。GAD-7的ROC曲线下面积(AUC)为0.84(95%CI:0.76~0.92),验证性因子分析显示该量表确为一因子结构,即焦虑因子(解释方差59.56%)。结论:中文版GAD-7在综合医院住院患者应用中具有良好的信效度,适用于该人群焦虑障碍的筛查。Objective： To examine the reliability and validity of a Chinese version of generalized anxiety disorder（ GAD-7） scale among hospitalized patients from general hospitals. Method： The mini-international neuropsychiatric interview（ MINI） was used as the gold standard,the validity of GAD-7 Chinese version was investigated in this study. A total of 569 inpatients from West China Hospital of Sichuan University were evaluated using the GAD-7,40 of them were randomly selected to retest using the GAD-7 on the 10 th day after the first evaluation and 150 participants with different scale of score were included consecutively to undertake the MINI.SPSS 18. 0 was used for data analysis. Results： The Cronbach＇s coeficient of GAD-7 was 0. 90 and the test-retest reliability was 0. 76. The range of correlation coefficient within 7 items was 0. 483-0. 713（ P〈0. 01） and the range between items and the total score of the scale was 0. 734-0. 820（ P〈0. 01）. The results of ROC analysis indicated that the area under the curve（ AUC） was good for the generalized anxiety disorder（ AUC =0. 84）. A cutoff score of 9,maximizing the Youden Index,yielded a sensitivity of 66. 70% and a specificity of85. 80%（ 95% CI： 0. 76-0. 92）. The scale was confirmed to be one-factor structure through exploratory factor analysis（ explained variance = 59. 56%）. Conclusion： The Chinese version GAD-7 is a reliable and valid measure to screen anxiety in general hospital inpatients.
