精神活性物质所致精神障碍患者肝功能损害的相关因素分析

摘　　要：目的:探讨精神活性物质所致精神障碍患者肝功能损害的相关因素。方法:将64例精神活性物质所致精神障碍住院患者(研究组)治疗前和保肝降酶治疗4周后的肝功能各项指标与55名健康人(健康对照组)进行比较,并对肝功能损害影响因素进行相关分析。结果:研究组治疗前肝功能各项指标高于健康对照组,其中谷丙转氨酶(ALT)、谷草转氨酶(AST)与对照组比较差异有统计学意义(t=4.063,t=3.044;P均<0.01);治疗后研究组肝功能各项指标均下降,与健康对照组比较差异无统计学意义。相关分析发现,ALT、AST与年龄(r=0.425,r=0.331)、首次住院年龄(r=0.268,r=0.262)、毒品使用年限(r=0.428,r=0.259)正相关(P<0.05或P<0.01);与受教育年限(r=-0.310,r=-0.371)负相关(P<0.05);ALT与住院次数(r=0.262)、总蛋白、球蛋白与住院次数(r=0.513,r=0.522)、毒品使用年限(r=0.296,r=0.419)正相关(P<0.05或P<0.01)。结论:精神活性物质所致精神障碍患者肝功能损害明显;年龄、文化程度、住院次数、毒品使用年限与肝功能损害显著相关。Objective： To investigate the related factors of liver function impairment in patients with mental disorders due to use of psychoactive substances. Method： The liver function indexes of 64 inpatients with mental disorders due to use of psychoactive substances（ study group） before treatment and those after protecting liver and decreasing enzyme for four weeks were compared with those of 55 healthy people（ healthy control group）,and the influencing factors of liver function impairment were analyzed. Results： The liver function indexes of the study group before treatment were higher than those of the control group,and the differences between alanine aminotransferase（ ALT） and aspartate aminotransferase（ AST） were statistically significant（ t =4. 063,t = 3. 044; all P〈0. 01）. After treatment,the liver function indexes of the study group decreased,and the difference was not statistically significant compared with the healthy control group. Correlation analysis showed that ALT and AST were positively related to age（ r = 0. 425,0. 331）,first hospitalization age（ r = 0. 268,0. 262）and drug use years（ r = 0. 428,0. 259; P〈0. 01 or P〈0. 05）,and negatively related to educational level（ r =-0. 310,-0. 371）,ALT was positively related to hospitalization times（ r = 0. 262）,total protein and globulin were positively related to hospitalization times（ r = 0. 513,0. 522） and drug use years（ r = 0. 296,0. 419; P〈0. 01 or P〈0. 05）. Conclusion： The impairment of liver function in patients with mental disorders due to use of psychoactive substances is obvious. Age,education level,times of hospitalization and drug use years are significantly related to liver function impairment.
