摘 要:目的探究银杏叶提取物(EGb)漱口前后单纯性龈炎患者菌斑中变形链球菌和牙龈卟啉单胞菌的变化以及患者牙周病学常用指数的变化。方法 2016年7月至2017年10月从哈尔滨医科大学的大学生中选取患有单纯性龈炎且均能检测出变形链球菌和牙龈卟啉单胞菌的60名志愿者,按照简单随机抽样法分为试验组和对照组,各30例。试验期间采用单盲法,对照组采用蒸馏水漱口;试验组采用EGb溶液漱口,受试者每天早、中、晚饭后各漱口一次,每次20 m L,连续2周。分别于漱口前、漱口后7 d、14 d取菌斑样本,同时检查和记录两组患者含漱前后的牙龈指数、菌斑指数及出血指数。采用实时荧光定量聚合酶链反应(q PCR)检测菌斑样本中变形链球菌和牙龈卟啉单胞菌的含量。结果应用EGb漱口14 d后,试验组牙龈指数呈明显下降趋势,两组时点间、组间·时点间比较差异有统计学意义(P<0.05),组间比较差异无统计学意义(P>0.05)。q PCR结果显示,应用EGb漱口14 d后,试验组菌斑中牙龈卟啉单胞菌的相对表达量呈下降趋势,两组组间、组间·时点间比较差异有统计学意义(P<0.05),不同时点间比较差异无统计学意义(P>0.05)。结论应用EGb漱口后,单纯性龈炎患者牙龈指数降低,菌斑中牙龈卟啉单胞菌的含量减少,变形链球菌的含量无变化。Objective To explore the changes of Streptococcus mutans and Porphyromonas gingivalis in the plaque of patients with simple gingivitis and the common periodontics index after gargling with extract of ginkgo biloba(EGb). Methods According to the inclusion criteria,we selected 60 volunteers from the students of Harbin Medical University with simple gingivitis and detected to have Streptococcus mutans and Porphyromonas gingivalis in bacterial plaque samples from Jul. 2016 to Oct. 2017.Then the 60 volunteers were divided into a test group and a control group by simple random sampling,30 cases each. Single blind method was adopted,subjects in control group rinsed their mouth with distilled water,while subjects in experimental group rinsed their mouth with EGb solution,and all the subjects rinsed their mouth after every meal in the morning, noon and evening,20 mL each time for 2 weeks. Samples of the plaque were taken before the test,and 7 d and 14 d after the test. Meanwhile,the gingival index,plaque index and bleeding index were examined and recorded in both groups.Real-time fluorescence quantitative polymerase chain reaction(qPCR) was used to detect the content of Streptococcus mutans and Porphyromonas gingivalis in bacterial plaque samples. Results After 14 d of application of EGb,the gingival indexes of the test group showed a significant decrease,and the differences between time points,group time points were statistically significant( P 〈0.05). At the same time,there was no statistically significant differences between the two groups( P 〉0.05).qPCR results showed that after 14 d of the application of EGb gargle,the relative expression of porphyromonas gingivalis in the test group decreased,the differences between groups, group time point were statistically significant( P 〈0.05),while the differences between different time points were not statistically significant( P 〉0.05).
