Lp-PLA2与H型高血压缺血性脑卒中的相关性研究

摘　　要：目的观察H型高血压缺血性脑卒中患者血清中脂蛋白相关性磷脂酶A2(Lp-PLA2)、同型半胱氨酸(Hcy)、超敏C反应蛋白(hs-CRP)、血脂等检测指标的变化,以明确Lp-PLA2与H型高血压缺血性脑卒中的相关性。方法选取2015年1月至2017年6月昆明医科大学第二附属医院门诊和住院患者,H型高血压缺血性脑卒中组103例,H型高血压组124例,原发性高血压组80例,体检健康者50例作为对照组,分别检测并比较各组血清中Lp-PLA2、Hcy、hs-CRP、血脂水平,采用二元Logistic回归分析法分析其与缺血性脑卒中的相关性。结果 H型高血压组及H型高血压缺血性脑卒中组患者血清总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)、LP-PLA2水平高于对照组和原发性高血压组,差异有统计学意义(P<0.05)。H型高血压缺血性脑卒中组患者血清三酰甘油(TG)水平高于对照组和原发性高血压组,差异有统计学意义(P<0.05)。原发性高血压组、H型高血压组及H型高血压缺血性脑卒中组患者血清Hcy水平高于对照组,差异有统计学意义(P<0.05);H型高血压组及H型高血压缺血性脑卒中组患者血清Hcy水平高于原发性高血压组,差异有统计学意义(P<0.05);H型高血压缺血性脑卒中组患者血清Hcy、LP-PLA2水平高于H型高血压组,差异有统计学意义(P<0.05)。H型高血压组患者血清hs-CRP水平高于对照组和原发性高血压组,差异有统计学意义(P<0.05);H型高血压缺血性脑卒中组患者血清hs-CRP水平高于对照组、原发性高血压组、H型高血压组,差异有统计学意义(P<0.05)。二元Logistic回归分析,结果显示Lp-PLA2与H型高血压缺血性脑卒中明显相关(SE=0.013,P<0.05)。结论 LP-PLA2是反应血管炎症的标志物之一,与H型高血压缺血性脑卒中的发生、发展密切相关。Objective To observe the change of results of lipoprotein-associated phospholipase A2（LpPLA2）,homocysteine（Hcy）,high-sensitivity C-reactive protein（hs-CRP）,lipids and other indicators in the patients with normal group,primary hypertension group,type H hypertension group and type H hypertension ischemic stroke group,and search the relationship between LP-PLA2 and type H-hypertension ischemic stroke.Methods From January 2015 to June 2017,continuous selected 103 patients with type H hypertension ischemic stroke group,124 patients with type H hypertension group,80 patients with primary hypertension group and 50 patients with healthy controls as normal group.Analyzed level of Lp-PLA2,Hcy,hs-CRP and lipids in serum,compared the difference with each group.A binary Logistic regression analysis was used to analyze its correlation with ischemic stroke.Results The serum concentration of total cholesterol（TC）,low-density lipoproteincholesterol（LDL-C）,LP-PLA2 in type H hypertension group and type H hypertension ischemic stroke group was higher than primary hypertension group and normal group,the difference was statistically significant（P〈0.05）.The serum concentration of triglyceride（TG）in type H hypertension ischemic stroke group was higher than primary hypertension group and normal group,the difference was statistically significant（P〈0.05）.The serum concentration of Hcy in primary hypertension group,type H hypertension group and type H hypertension ischemic stroke group was higher than normal group,the difference was statistically significant（P〈0.05）;The serum concentration of Hcy in type H hypertension group and type H hypertension ischemic stroke group was higher than primary hypertension group,the difference was statistically significant（P〈0.05）;The serum concentration of Hcy,LP-PLA2 in type H hypertension ischemic stroke group was higher than type H hypertension group,the difference was statistically significant（P〈0.05）.The serum concentration of hs-CRP in type H hype
