摘　　要：目的比较分析腹腔镜直肠癌Mile's术中乙状结肠经腹膜外造瘘与经腹膜内造瘘的临床效果。方法选取2012年6月至2017年6月江苏大学附属医院收治的行腹腔镜Mile's术直肠癌患者73例,根据造瘘方式不同将患者分为两组,腹膜外造瘘组35例给予经腹膜外造瘘术,腹膜内造瘘组38例给予经腹膜内造瘘术,观察并比较两组患者术中和术后恢复情况,术后并发症发生情况。结果与腹膜内造瘘组相比,腹膜外造瘘组术后排气、术后排便时间较短,术后造口旁疝发生率较低,差异均有统计学意义(均P<0.05)。结论腹腔镜直肠癌Mile's术乙状结肠经腹膜外造瘘较经腹膜内造瘘的术后情况明显改善,且并发症减少,值得临床应用。Objective To investigate the clinical effects of extraperitoneal colostomy andintraperitoneal colostomy in laparoscopic Mile＇s surgery for rectal cancer. MethodsA total of 73 rectal cancerpatients who were treated with laparoscopic mile＇s surgery from June 2012 to June 2017 in the Affiliated Hospitalof Jiangsu University were selected and divided into two groups. Thirty-five patients in the experimental groupwere treated by extraperitoneal colostomy, while another 38 patients in the control group were treated by intrap-eritoneal colostomy. Then the intraoperative and postoperative recovery and complications of patients in the twogroups were observed and compared. ResultsCompared with the control group, the exhaust and defecationtime in the experimental group were lower （P〈0. 05）, and the incidence rate of parastomal hernia in the exper-imental group was lower （P〈0. 05）. ConclusionsThe clinical effect of extraperitoneal sigmoid colostomy wasbetter than that of the intraperitoneal sigmoid colostomy in laparoscopic mile＇s surgery for rectal cancer, whichhas less complication and is worth of clinical application.
