摘 要:目的探讨血清miRNA复发分子标志物检测与鼻咽癌患者出现复发远处转移相关性。方法选取病理确诊的鼻咽鳞癌患者60例行8 Me V高能X射线调强放射治疗和奈达铂联合氟尿嘧啶的化疗方案。治疗结束后每3个月对患者随访1次。随访29~65个月,经MRI和病理检测25例没有发生远处转移和复发(未复发转移组),35例发生远处转移或复发(复发转移组),应用miRCURY LNATMmicroRNA Arrays芯片对血清内miRNA差异性表达谱筛查,免疫组织化学检测患者肿瘤组织内Jab1与Akt1表达情况。结果 miRNAs芯片结果显示,治疗前复发转移组比未复发转移组血清内miRPlus E1253的表达量上升,miR-125b、miR-22、miR-26b、miR-29a、miR-665和miR-720的表达量下降(均P<0.05)。治疗后复发转移组比未复发转移组血清内miRPlus E1072的表达量上升,miR-26b、miR-122、miR-30c、miR-29b、miR-550、miR-143、miR-720和let-7c的表达量下降(均P<0.05)。未复发转移组患者肿瘤组织内Jab1与Akt1蛋白阳性表达率均低于复发转移组(均P<0.05)。Logistic回归分析显示,治疗前血清miR29b、miR125b表达量和Jab1、Akt1阳性表达为患者复发转移独立危险因素(均P<0.05)。结论血清miR-125b、miR29b水平及肿瘤组织内Jab1与Akt1阳性表达与鼻咽癌患者远处转移或复发相关。Objective To investigate the correlation between serum miRNA biomarkers and the recurrence and distant metastasis in nasopharyngeal carcinoma( NPC) patients. Methods Sixty patients with pathologically confirmed NPC were enrolled in the study. The patients were treated with 8 Me V high-energy X-ray intensity modulated radiation therapy( IMRT),as well as nedaplatin combined with 5-fluorouracil( 5-FU) chemotherapy. Patients were followed up every 3 months after treatment for 29-65 months. MRI and pathological tests found no distant metastases or recurrences in 25 cases( non-relapsed group),and distant metastases or recurrences in 35 cases( recurrence and metastasis group). Serum miRNAs were detected by miRCURY LNA^TM microRNA arrays. The expression of Jab1 and Akt1 in tumor tissue was detected by immunohistochemistry. Results The miRNAs microarray showed that the expression of miRPlus E1253 was increased,whereas the expression of miR-125 b,miR-22,miR-26 b,miR-29 a,miR-665,miR-720 was decreased in the recurrence and metastasis group before the treatment,as compared with the non-relapsed group( all P〈0. 05). After the treatment,the expression of miRPlus E1072 was increased in the recurrence and metastasis group compared with the non-relapsed group,whereas the levels of miR-26 b,miR-122,miR-30 c,miR-29 b,miR-550,miR-143,miR-720,and let-7 c were decreased in the recurrence and metastasis group( all P〈0. 05). Moreover,the positive expression rates of Jab1 and Akt1 protein in the tumor tissue were lower in the non-relapsed group than those in the recurrence and metastasis group( both P〈0. 05).Finally,Logistic regression analysis showed that pretreatment serum miR29 b and miR125 b expression and the positive expression of Jab1 and Akt1 were independent risk factors for recurrence and metastasis. Conclusion Serum miR-125 b and miR29 b and the positive expression of Jab1 and Akt1 are associated with the distant metastasis or recurrence of NPC patients.
