改良型FRⅢ型矫治器矫治儿童安氏Ⅲ类错后软硬组织的改变

摘　　要：目的:探讨改良型FRⅢ型矫治器(MFRⅢ)矫治儿童功能性安氏Ⅲ类错后软硬组织的变化。方法:纳入儿童功能性安氏Ⅲ类错患者40例,随机分为2组(n=20),实验组采用MFRⅢ型矫治器矫治,对照组采用传统FRⅢ型矫治器矫治,对2组病例矫治前后的软硬组织位置进行对比分析。结果:实验组与其治疗前比较,SNA、ANB、ANS-Ptm、ANS-Me、U1-NA、FCA增大(P<0.01),MP-FH、NLA、UL-EP增大(P<0.05),ANS-Me/N-Me、LL-EP减小(P<0.01)。与对照组的软硬组织位置比较,SNA、ANB、ANS-Ptm、ANS-Me、L1-NB,NLA、增大(P<0.05),U1-NA、FCA、LL-Sb-Pos增大(P<0.01),MP-FH、LL-EP减小(P<0.05),矫治时间缩短(P<0.05)。结论:MFRⅢ型矫治器治疗儿童功能性安氏Ⅲ类错,可以使第一磨牙达到中性咬合,面型从凹面型趋于平直并与对照组比较,旋转角度有所减小,下前牙代偿性舌倾度减小、上颌长度增加,使上颌骨突度及矢状位面型的改善更为明显,并缩短了疗程。Objective： To study changes in soft and hard tissues after treatment of angle class Ⅲ malocclusion by modified appliance FRⅢ（ MFRⅢ）. Methods： 40 patients with Angle class Ⅲ malocclusion and deciduous dentition or mixed dentition were divided into2 groups（ n = 20） and treated by MFRⅢ and FRⅢ respectively. The soft and hard tissue position of the subjects were comparatively analyzed using X-ray cephalometrics before and after treatment. Results： After treatment SNA,ANB,ANS-Ptm,ANS-Me,U1-NA and FCA increased（ P 〈 0. 01）,MP-FH,NLA and UL-EP increased（ P 〈 0. 05）,ANS-Me/N-Me and LL-EP decreased（ P 〈 0. 01） in group MFRⅢ. Compared with those in FRⅢ group,SNA,ANB,ANS-Ptm,ANS-Me,L1-NB and NLA increased（ P 〈 0. 05）,U1-NA,FCA and LL-Sb-Pos increased（ P 〈 0. 01）,MP-FH and LL-EP decreased（ P 〈 0. 05）,the treatment time decreased（ P 〈 0. 05） in group MFRⅢ. Conclusion： MFRⅢ is effective in the correction of anterior crossbite and first molar occlusion,MFRⅢ can make facial shape from concave to flat and straight. Although MFRⅢ can not effectively avoid the adverse effects of clockwise rotation of mandible,but can decrease the rotation angle,can reduce the compensatory lingual inclination of the lower anterior teeth,with the increase of maxillary length,and can improve maxillary protrusion and sagittal facial type,shorten the treatment time.
