已修复唇腭裂与骨性Ⅲ类成人患者颌骨差异

摘　　要：目的:探讨已唇腭裂手术修复后的单侧完全性唇腭裂(complete unilateral cleft lip and palate,UCLP)男性成人患者与骨性Ⅲ类男性成人上下颌骨位置与形态的差异。方法:纳入36例UCLP男性成人患者及120例非裂健康人群骨性Ⅲ类男性成人;用Wincep 8.0软件对所有样本的头颅侧位片进行头影测量;使用独立样本t检验对不同组头影测量项目统计分析。结果:已唇腭裂手术修复后的UCLP男性成人患者的SNA角、SNB角、SND角、Go-Gn距均值较骨性Ⅲ类患者明显减小,腭平面角(SN-PP)、下颌平面角(SN-MP),面下高度(ANS-Me)、U1-SN角较骨性Ⅲ类患者明显增大(P<0.05)。结论:与骨性Ⅲ类男性成人相比已修复UCLP成人男性患者的上下颌位置较靠后,下颌体长度减小,腭平面顺时针旋转,下颌平面角更高,面下高增大,上前牙更为直立甚至舌倾。Objective： To investigate the difference of location and morphology of maxilla and mandibule between adult male patients with complete unilateral cleft lip and palate（ UCLP） patients after anaplasty and those with skeletal class Ⅲ malocclusion. Methods：The sample consisted of 36 patients with UCLP after anaplasty and 120 patients with skeletal class Ⅲ malocclusion. Lateral cephalograms of all subjects were obtained by winceph 8. 0 software. Independent sample t test was used to detect the difference between the 2 groups. Results： The adult UCLP group showed significantly smaller values for SNA angle,SNB angle,SND angle,Go-Gn length and inclination of the maxillary incisor as compared with those of adult skeletal class Ⅲ malocclusion patients. The values of SN-PP angle,SN-MP angle,lower face height and U1-SN angle in the adult male UCLP group were significantly bigger than those with skeletal class Ⅲ malocclusion. Conclusion： UCLP patients have the retracted maxilla and mandible compared with the patients with skeletal class Ⅲ malocclusion,the length of the mandibular body is smaller,the plane of the palate is clockwisely rotated,the mandible plane was steeper,and the anterior teeth are more upright.
