血清CA125、HE4及ROMA指数在卵巢癌中的诊断价值及HE4新参考区间设立的研究

摘　　要：目的:研究血清人附睾蛋白4(HE4)、CA125和卵巢癌风险预测(ROMA)指数在卵巢癌中的诊断价值并设立新的HE4参考值。方法:选取卵巢癌患者70例,对比同期收治的卵巢良性包块患者的血清HE4、CA125及ROMA值的表达水平,对比绝经前后表达水平的差异,描绘相应的ROC曲线,建立新的HE4参考区间,并对新参考区间的检验效能进行验证。结果:(1)对比良性卵巢包块,血清HE4、CA125及ROMA值在卵巢癌中表达水平更高,差异有统计学意义(P<0.05)。(2)整体上,血清CA125、HE4及ROMA的ROC-AUC分别是0.755、0.830、0.833。(3)血清HE4的参考区间为<59 pmol/L。结论:HE4联合CA125取得的ROMA值诊断卵巢癌效能更高,优于单独检测。设立HE4值的参考区间有助于提高卵巢癌诊断的准确性。本文设立的新的HE4参考区间更适用于本地区人群。Objective To investigate the diagnostic value of human epididymis protein 4（HE4）,CA125 and the risk of ovarian malignancy algorithm（ROMA） index in ovarian cancer and establish a new HE4 reference value.Method 70 patients with ovarian cancer were selected to compare the expression of HE4,CA125 and ROMA in patients with benign ovarian mass in the same period.And compare the difference of the expression level before and after menopause,describe the corresponding ROC curve,establish a new HE4 reference interval,and verify the efficacy of the new reference interval.Results（1）Compared with benign ovarian masses,the serum levels of HE4,CA125 and ROMA were higher in ovarian cancer（P0.05）.（2）On the whole,the ROC-AUC of serum CA125,HE4 and ROMA were 0.755,0.830 and 0.833,respectively.（3）The reference range of serum HE4 was less than 59 pmol/L.Conclusion The ROMA value obtained by HE4 combined with CA125 is more effective in the diagnosis of ovarian cancer,which is better than the single test.The reference interval of setting up the HE4 value can help to improve the accuracy of the diagnosis of ovarian cancer.The new HE4 reference interval established in this paper is more suitable for the local population.
