改良常温流动牙胶根管充填技术的临床评价

摘　　要：目的评价改良的常温流动根管充填技术的临床疗效。方法选择2017年4~7月因各型牙髓炎或根尖周炎来南京市口腔医院牙体牙髓科就诊的80例患者的88颗患牙,按随机数表法分为试验组和对照组,每组44颗牙,所有牙均采用两次法完成根管治疗。试验组以改良法Gutta Flow+单牙胶尖充填根管,对照组以AH plus根管封闭剂+热牙胶连续波技术充填根管。比较两种根管充填方法的充填时间、术中及术后的疼痛发生率、根管充填的恰填率和超填率。结果试验组患者的每根管充填时间为40~69 s(平均48 s),明显少于对照组的52~111 s(平均76 s),试验组患者的术中及术后疼痛发生率分别为4.55%和15.91%,明显低于对照组的34.09%和36.37%,以上各项指标比较差异均有统计学意义(P<0.05);试验组患者的根管充填恰填率和超填率分别为95.83%和2.08%,分别与对照组的91.84%和7.14%比较差异均无统计学意义(P>0.05)。结论改良的常温流动牙胶根管充填技术充填时间短,术中及术后疼痛发生率低,充填质量与热牙胶连续波充填相近,具有一定的临床应用价值。Objective To evaluate the clinical effect of modified root canal filling technique with Gutta Flow.Methods A total 88 teeth of 80 patients, who admitted to the Department of Endodontics of Nanjing Stomatological Hospital due to pulpitis or periapical periodontitis from April 2017 to July 2017, were selected and divided into the experimental group and the control group according to random number table, with 44 cases in each group. The root canal treatment was completed by two visits. The root canal was filled with modified method of Gutta Flow in the experimental group, while it was filled with AH plus ＋ warm gutta-percha in the control group. The time, the intraoperative and postoperative pain rate and the quality of root canal filling were compared between the two groups. Results The filling time of each root canal in the experimental group was 40~69 s（mean 48 s）, which was significantly less than 52~111 s in the control group（mean 76 s）, P〈0.05. The incidence of pain in the experimental group was 4.55% and 15.91%, respectively, which was significantly lower than 34.09% and 36.37% in the control group（P〈0.05）. The filling rate and the excess filling rate of the root canal filling in the experimental group were 95.83% and 2.08%, respectively, as compared with 91.84% and 7.14% in the control group（P〉0.05）. Conclusion The modified root canal filling technique with Gutta Flow had advantages of simple operation, low intraoperative and postoperative pain rate and good quality of filling. It had certain clinical application value.
