p16,Ki67,CK8、17在CIN及宫颈鳞癌中的临床表达及病理意义

摘　　要：目的探究p16,Ki67,细胞角蛋白8、17在宫颈上皮内瘤变及浸润性鳞状细胞癌中的临床表达及其病理意义。方法选取250例宫颈活检患者,采用免疫组织化学技术法检测宫颈上皮内瘤变125例(低级组别60例,高级组别65例)、浸润性鳞性细胞癌65例、正常宫颈组织60例中p16、Ki67、细胞角蛋白8、17的阳性表达率。结果 p16,Ki67,CK8、17在正常宫颈对照组均为阴性表达;在浸润性鳞状细胞癌中的阳性表达率分别为100.0%、100.0%、89.2%、93.8%;在宫颈上皮内瘤变高级别中的阳性表达分别为100.0%、98.5%、56.9%、58.5%;在宫颈上皮内瘤低级别中的阳性表达率分别为63.3%、62%、13.3%、31.7%。结论 p16,Ki67,细胞角蛋白8、17的阳性表达率与宫颈病变严重程度成正比,是判断宫颈上皮内瘤病变、浸润性鳞状细胞癌增殖状态的有效指标。Objective To investigate the clinical expression of p16,Ki67 and cytokeratin 8 and 17 in cervical intraepithelial neoplasia and invasive squamous cell carcinoma and its pathological significance. Methods 250 cases of cervical biopsy were selected. 125 cases of cervical intraepithelial neoplasia（ 60 cases of low group,65 cases of advanced group）,65 cases of invasive squamous cell carcinoma and 60 cases of normal cervical tissue were detected by immunohistochemical technique. The positive expression rate of p16,Ki67 and cytokeratin 8,17 in the cases. Results The positive expression rates of p16,Ki67,CK8,17 in normal cervical control group were 100%,100%,89. 2% and 93. 8% respectively in invasive squamous cell carcinoma. The positive expression rates were 100%,98. 5%,56. 9% and 58. 5% in cervical intraepithelial neoplasia,respectively. The positive expression rates were 63. 3%,62%,13. 3%,31. 7% in cervical intraepithelial neoplasia. Conclusion The positive expression rate of p16,Ki67 and cytokeratin 8 and 17 is proportional to the severity of cervical lesions,which is an effective index to judge the proliferation of cervical intraepithelial neoplasia and invasive squamous cell carcinoma.
