超强纤维带和维他灵支架牙周夹板联合应用在游离松动牙固定中的效果评价

摘　　要：目的:评价超强纤维带+维他灵支架牙周夹板联合应用对重度牙周炎松动牙的固定效果。方法:选取牙列缺损并存在游离松动牙的重度牙周炎患者28例,随机分为实验组和对照组(n=14),实验组行纤维带固定松动牙后配戴维他灵支架牙周夹板;对照组单纯以纤维带固定松动牙。检查并记录治疗前及治疗后1年患牙松动度、牙周探诊深度(PD)、附着丧失(AI),并结合患者主观感觉来评价疗效。结果:治疗后1年,实验组和对照组的牙齿松动位移值、PD、AI与治疗前相比均有显著差异(P<0.05);组间相比,患牙松动位移值实验组明显低于对照组(P<0.05),而PD及AI则无显著差异(P>0.05);两组临床有效率均为92.86%,但实验组疗效满意度中的良好率高于对照组(P<0.05)。结论:采用超强纤维带+维他灵支架牙周夹板对有牙列缺损并存在游离松动牙的重度牙周炎进行松动牙固定,临床检查及患者评价均有良好效果,对牙齿松动度的改善优于单纯超强纤维袋固定法。AIM： To evaluate the fixation effect of the combined method of ultrastrong fibrous band plus vitallium framework and periodontal splint and loop method with single ultrastrong fibrous band on the loose teeth for patients with advanced and severe periodontitis. METHODS： 28 cases of patients with chronic and severe periodontitis were were assigned to the combination group and fiber group （ n =14）. In the combination group, after the fixation with the fibrous band for loose teeth was performed, the vitallium framework and periodontal splint were worn; in the fiber group, the conventional fixation with the fibrous band for loose teeth was performed. before and after the treatment the tooth mobility, the periodontal probing depth（PD） and attachment loss（AI） were measured and compared. RESULTS： After the treatment tooth mobility, PD and AI in both groups were decreased（ P 〈0.05）; and in the combination group were significantly lower than those in the fiber group（ P 〈0.05）. The clinical efficacy rate for both groups was 92.86% one year after the treatment, however the clinical satisfaction in the combination group was higher than that for the fiber group（ P 〈0.05 ）. CONCLUSION： The combination method of ultrastrong fibrous band plus vitallium framework and periodontal splint is effective for the fixation of loose teeth in patients with advanced and severe periodontitis.
