表面麻醉在经直肠前列腺活检术中对疼痛控制的疗效观察

摘　　要：目的观察表面麻醉在经直肠前列腺活检术中对疼痛的控制效果。方法将100例在成都军区总医院超声科经直肠前列腺活检的患者随机分为两组,试验组术前2~3min将盐酸丁卡因胶浆涂抹在肛周皮肤及注入肛门内,再常规进行活检;对照组按常规进行活检术。术后通过视觉模拟评分(VAS)进行疼痛评分,观察两组术中疼痛控制效果、并发症的发生率。结果穿刺过程中,试验组疼痛评分为(3.34±2.44)分,对照组为(5.18±1.98)分,试验组中度、重度疼痛的患者[11(22%)、6(12%)]少于对照组[29(58%)、12(24%)],差异有统计学意义(P<0.05)。试验组手术时间少于对照组,差异有统计学意义(P<0.05)。两组探头置入肛门时疼痛评分及术后并发症发生率差异无统计学意义(P>0.05)。结论表面麻醉在经直肠前列腺活检术中对穿刺时的疼痛控制具有较好的效果,其操作简单、方便、并发症少,值得临床推广应用。Objective To observe the curative effect of topical anesthesia on pain control in transrectal prostate biopsy. Methods A total of 100 patients undergoing the transrectal prostate biopsy in Chengdu Military General Hospital were divided randomly into two groups. In the test group, the tetracaine hydrochloride glue was smeared into the perianal skin and injected into the anus 2 to 3 minutes before operation and then the biopsy was performed. In the control group, the routine biopsy was performed. The pain was scored by visual analogue score （VAS）. The effect of pain control and the incidence of complications were observed in both groups. Results The intraoperative pain score was （3.34±2.44） in the test group and （5.18±1.98） in the control group. The numbers of patients with moderate and severe pain in the test group were 11 （22%） and 6 （12%） respectively, which were significantly lower than those in the control group which were 29 （58%） and 12 （24%） respectively. The operation time of the test group was significantly less than that of the control group （ P 〈0.05）. There were no significant differences in the pain score when the probe was inserted into the anus and the postoperative complications. Conclusion Topical anesthesia has a good effect on pain control in transrectal prostatic biopsy and it is simple and convenient with less complications, so it is worth the clinical promotion.
