摘　　要：目的检测甲状腺髓样癌(medullary thyroid carcinoma,MTC)中Rb和CyclinD1蛋白表达情况,分析其与患者预后的关系。方法选取四川大学华西医院2010年1月至2016年12月手术切除确诊为MTC并有随访资料的患者106例,采用免疫组化S-P法检测Rb和CyclinD1蛋白的表达情况。统计分析Rb和CyclinD1蛋白表达的相关性及其与患者临床病理特征和预后的关系。结果在MTC样本中,Rb蛋白低表达与髓样癌患者性别、年龄、肿瘤直径、包膜侵犯、淋巴结转移及TNM分期无关,而与脉管内癌栓形成有关(P<0.05);CyclinD1阳性表达与肿瘤直径、淋巴结转移和临床病理分期有关(P<0.05)。预后分析显示,Rb蛋白低表达是髓样癌预后不良的因素(P<0.05)。两者表达呈正相关(r=0.453,P<0.05)。结论 Rb蛋白表达情况可作为髓样癌总生存率的预测指标。Objectives To detect the expression of Rb and CyclinD1 in medullary thyroid carcinoma （MTC） and analyze the relationship between Rb, CyclinD1 and patients＇ prognosis. Methods A total of 106 MTC cases diagnosed and treated in West China Hospital of Sichuan University with the follow up data from 2010 to 2016 were enrolled in the stuy. The expression of Rb and CyclinD1 protein were detected by immunohistochemical S-P staining. The correlation between Rb and CyclinD1 protein expression and their relationship with clinicopathological characteristics and patients＇ prognosis. Results In the MTC samples, the low expression of Rb protein in MTC was not correlated with gender, age, diameter of tumor, capsular invasion, lymph node metastasis and TNM stage, while it was significantly correlated with intravascular tumor thrombus （P〈0.05）. The positive expression of CyclinD1 in MTC was significantly correlated with the tumor diameter, clinicopathologic stage and lymph node metastasis （P〈0.05）. The prognosis analysis showed that the low expression of Rb protein was the prognostic factor for medullary carcinoma （P〈0.05） and there was a positive correlation The expression of Rb protein can be used as a predictor of overall survival of medullary carcinoma.
