进阶式培训模式在前列腺癌根治术中的价值和临床应用

摘　　要：目的评估进阶式培训模式在前列腺癌根治术中的价值和临床应用。方法进阶式培训模式分为手术分解培训和手术操作培训,将手术分为盆腔淋巴结活检、前列腺及精囊的切除和膀胱颈尿道的吻合3个部分,手术操作培训分为2个阶段,包括腹腔镜操作阶段和机器人辅助腹腔镜操作阶段。手术前观看前列腺癌根治术经典录像,术后观看当天手术视频并进行比较,最后共同讨论分析总结。结果 2017年1—10月,共完成腹腔镜前列腺癌根治术16例,机器人辅助前列腺癌根治术12例。2个阶段中,培训医师的手术时间明显缩短,出血量显著减少,无中转开放,术后患者恢复良好。结论通过进阶式机器人腹腔镜前列腺癌根治术的培训,可以快速缩短学习曲线,在更短时间内掌握微创操作技能。Objective This paper tries to evaluate the value and clinical applications of the progressive training model in the radical prostatectomy. Methods The advanced training model was divided into surgical decomposition training and surgical operation training. The surgery was divided into pelvic lymph node biopsy, resection of prostate and seminal vesicle,and bladder neck urethra anastomosis. The operation training training was divided into two phases, including laparoscopic operation phase and robotic assisted laparoscopic procedures. Before surgery, see the classic video of prostate cancer radical surgery, watch the video of the day after surgery and compare them. Finally, discuss and summarize the results. Results From January to October 2017, 16 cases of laparoscopic radical prostatectomy and 12 cases of robotic assisted radical prostatectomy were performed. In the two phases, the training time for physicians was significantly shortened, the amount of bleeding was significantly reduced, there was no transfer to open, and patients recovered well after surgery. Conclusion By training advanced robotic laparoscopic radical prostatectomy, the learning curve can be quickly shortened and the minimally invasive operation skills can be mastered in a shorter period of time.
