ERCC2基因rs1799793多态性影响顺铂处理三阴性乳腺癌细胞增殖和凋亡的机制研究

摘　　要：目的探讨ERCC2基因rs1799793多态性位点的基因型与三阴性乳腺癌易感性以及铂类化疗方案敏感性之间的关系。方法以收治的40例拟行含顺铂药物化疗的三阴性乳腺癌患者为研究对象(观察组),并选取同期入院体检的40例健康人为正常对照(对照组),比较2组受试者ERCC2基因rs1799793多态性位点的基因型;分析三阴性乳腺癌患者ERCC2基因rs1799793多态性位点基因型与其一般资料、临床病例参数及化疗敏感性的关系。结果观察组受试者rs1799793位点GA基因型频率明显高于对照组(17.50%、12.50%),2组差异有统计学意义(P<0.05);疗效评价显示,携带GG基因型的三阴性乳腺癌患者化疗总有效率明显高于GA基因型携带者(66.67%、28.57%),ERCC2基因rs1799793位点不同基因型间化疗敏感性差异有统计学意义(P<0.05);ERCC2基因rs1799793位点基因型与患者年龄、家族史、肿瘤大小、病理类型以及TNM分期均无明显相关性(P>0.05)。结论ERCC2基因rs1799793位点GA基因型可能与患者三阴性乳腺癌易感性相关,且携带该基因型的三阴性乳腺癌患者铂类化疗敏感性较低。Objective To investigate the relationship between the genotype of the excision repair cross complementing2（ERCC2） gene rs1799793 polymorphic site and susceptibility of triple negative breast cancer as well as the and sensitivity of platinum type chemotherapy scheme. Methods A total of 40 patients with triple negative breast cancer who were ready to be treated by cisplatinum chemotherapy were enrolled as research objects（observation group）,at the same time,the 40 healthy subjects who received physical examination in our hospital were enrolled as control group. The genotypes of the ERCC2 gene rs1799793 polymorphic site in the two groups were observed and compared. The relationship between the genotype of the ERCC2 gene rs1799793 polymorphic site of patients and triple negative breast cancer and the general information,parameters of clinical case,the sensitivity of chemotherapy was analyzed. Results The frequency of GA genotype in rs1799793 site（17. 50%） in observation group was significantly higher than that（12. 50%） in control group（P〈 0. 05）. Therapeutic effects evaluation showed that the total effective rate of chemotherapy in triple negative breast cancer patients with GG genotype was 66. 67%,which was significantly higher than that（28. 57%） in the patients with GA genotype（P〈 0. 05）. Moreover there was significant difference in chemotherapy sensitivity of patients with different genotypes in ERCC2 gene rs1799793 site（P〈 0. 05）. The genotype of the ERCC2 gene rs1799793 site was not correlated with patient＇s age,family history,tumor size,pathological pattern and TNM stage（P〉0. 05）. Conclusion The GA genotype in the ERCC2 gene rs1799793 site may be related to the susceptibility of patients with triple-negative breast cancer,moreover the sensitivity of platinum chemotherapy in triple negative breast cancer patients with GA genotype is lower.
