摘 要:(1)目的探讨P16与Ki-67在宫颈鳞状上皮内病变中的表达及应用。(2)方法收集宫颈活检标本共计262例,其中宫颈炎(对照组)组117例,低度鳞状上皮内病变(LSIL)组87例,高度鳞状上皮内病变(HSIL)组51例,鳞状细胞癌(SCC)组7例,应用免疫组化方法检测P16与Ki-67在各组病变中的表达情况。(3)结果 P16与Ki-67在SCC组、HSIL组及LSIL组中的阳性率均显著高于宫颈炎组(P<0.05);在SCC组及HSIL组中的阳性率均显著高于LSIL组(P<0.05);SCC组与HSIL组之间无显著差异;且P16阳性率与Ki-67阳性率呈正相关(r=0.908,P<0.05);Ki-67在HSIL及SCC组中以分布于上皮中下2/3直至全层为主,LSIL组以分布于上皮下1/3为主。(4)结论免疫组化法检测P16与Ki-67在宫颈鳞状上皮内病变中的表达情况,可以对宫颈病变的准确分级起协助作用。Objective To investigation the expression and significance of P16 and Ki-67 in cervical squamous epithelial lesions.Methods Wecollected 262 cases of cervical tissues from surgical excision,including 117 cases of cervicitis(control group),87 cases of low grade squamous intraepithelial lesion(LSIL),51 cases of high grade squamous intraepithelial(HSIL),and 7 cases of cervical squamous cell carcinomal(SCC).The expression of P16 and Ki-67 in cervical tissue were detected by using immunohistochemistry.Results The positive expression rates of P16 and Ki-67 in squamous cell carcinomal、HSIL and LSIL group was significantly higher than those in cervicitis group(P 〈0.05);The positive expression rates of P16 and Ki-67 in squamous cell carcinomal and HSIL group was significantly higher than those in LSIL group(P 〈0.05);and there are no significantly difference between the squamous cell carcinomal group and HSIL group;The expression of P16 positively correlated with the expression of Ki-67(r =0.908,P 〈0.05);The expression of Ki-67 was more distributed in the location of lower1/3 in LSIL,but was more distributed in the location〉2/3,even the whole epithelium.Conclusion To assisting the accurate classification of cervical lesions,we can combine detection theexpression of P16 and Ki-67 in cervical squamous intraepithelial lesion.
