意向再植术治疗严重病变下颌第二磨牙疗效观察

摘　　要：目的:观察意向再植术治疗1例严重病变下颌第二磨牙的临床疗效。方法:将1例男性47大面积缺损、根尖周脓肿、牙周牙髓联合病变、48垂直阻生的患者47拔除后微创拔除48,即刻再植入47牙槽窝并钢丝固定,48行常规根管治疗术,术后3个月拆除钢丝。结果:术后拍摄X线片显示48复位良好;1个月后复查瘘道消失,牙齿无松动,X线片示根尖区新骨形成;术后6个月复查,X线片示根尖区新骨进一步形成,密度接近周围骨组织,X线片显示牙周间隙存在,无牙根外吸收。结论:意向再植术是通过拔除智齿再植来治疗磨牙严重病变的有效方法。Objective The clinical effect of 1 cases of mandibular second molar with severe lesions was treated by intentional replantation. Methods 47 molar with large area caries, periapical abscess, periodontal endodontic joint disease and 48 vertical impacted patientwas selected,minimally invasive removal of 48, immediate reimplantation of 47 alveolar fossa and steel wire fixation. Results Postoperative X-ray showed good reduction of 48; after 1 month the fistula disappeared, no loose teeth, X-ray showed apical area of new bone formation, 48 underwent conventional root canal therapy; postoperative March demolition wire; 6 months after operation by X-ray showed new bone formation in apical area further, density close to bone tissue. X-ray showed the presence of periodontal clearance, no external root resorption. Conclusion Intentional replantation is an effective method for the treatment of mandibular second molar with severe lesions by removing the reimplantation of the wisdom tooth.
