手助腹腔镜下巨大肾血管平滑肌脂肪瘤剜除术的应用

摘　　要：目的探讨手助腹腔镜下巨大(≥8cm)肾血管平滑肌脂肪瘤剜除术的应用价值和方法。方法回顾性分析本单位2015年4月至2017年3月期间行手助腹腔镜下巨大肾血管平滑肌脂肪瘤剜除术患者资料,记录手术时间、术中失血量、住院时间及短期随访结果。结果 14例患者均手术成功,术中无中转开放手术。手术时间100~240分钟,平均159分钟;术中出血100~800ml,平均277ml;术后住院时间3~12天,平均7.5天。结论手助腹腔镜下巨大肾血管平滑肌脂肪瘤剜除术创伤小、出血少、安全高效,患者恢复快,在术者手的触觉帮助下,可以完成较为复杂的肾血管平滑肌脂肪瘤剜除术手术。Objective To evaluate the effect and clinical application of hand-assisted laparoscopic for enucleation of giant（ ≥8 cm） renal angiomyolipoma. Methods The patients underwent hand-assisted laparoscopic for enucleation of giant renal angiomyolipoma in our hospital from April 2015 to March 2017 were reviewed. Results Allcases were performed successfully. The mean operation time was 159 minutes（ 100 - 240 minutes）,and the mean blood loss was 277 ml（ 100 - 800 ml）. The mean postoperative hospital stay was 7. 5 days（ 3 - 12 days） in all cases. Conclusions With the advantages of minimal invasion,less blood loss,faster recovery,hand-assisted laparoscopy for enucleation of giant（ ≥8 cm） renal angiomyolipoma is worth popularizing in urologic clinical practice,and with the help of the operator＇s hand,complex laparoscopic surgery can be performed.
