单、双相抑郁症临床特征及血清甲状腺激素水平比较研究

摘　　要：目的研究对比单、双相抑郁症患者的血清甲状腺激素水平,总结其临床特征。方法方便选取2016年1月—2017年12月该院收治的50例单相抑郁症视作单相组,50例双相抑郁症患者视作双相组,另外选取50名健康者作为对照组,3组均接受血清甲状腺激素水平测定,利用HAMD进行抑郁程度的测量。结果单相组初次发病年龄(37.56±12.45)岁,焦虑评分(10.85±2.33)分,认知评分(7.85±1.60)分,存在明显发病诱因占52%,均与双相组(29.34±12.59)岁、(10.39±2.22)分、(6.82±1.86)分、20.00%比较差异有统计学意义(t/χ2=3.265 7、3.258 4、3.058 7、7.452 6,P<0.05);双相抑郁症患者FT3、FT4指标测定结果高于单相患者;单相患者HAMD迟滞因子评分和血清T4指标水平存在相关性(P<0.05)。结论单、双相抑郁症患者在临床特征方面存在一定差异,双相抑郁症的生物学标记为血清FT4。Objective This paper tries to compare serum thyroid hormone levels in patients with single and bipolar depression and to summarize their clinical features. Methods From January 2016 to December 2017, 50 patients with unipolar depression in this hospital were treated as single-phase group, 50 patients with bipolar depression were considered as biphasic group, and 50 healthy subjects were selected as control group. Serum thyroid hormone levels were measured and HAMD was used to measure the degree of depression. Results The age of onset was（37.56±12.45） years old in the monophasic group,the anxiety score was（10.85±2.33）points, and the cognitive score was（7.85±1.60）points. There was a significant predisposing predisposition of 52.00%, both with the two-phase group（29.34 ±12.59） years old and（10.39±2.22） points,（6.82±1.86）points, 20.00%. There was a significant difference（t/χ^2=3.265 7, 3.258 4, 3.058 7, 7.452 6, P〈0.05）; the results of FT3 and FT4 indexes in patients with bipolar depression were higher than monophasic patients; there was a correlation between HAMD lag factor score and serum T4 index level in patients with single phase（P〈0.05）. Conclusion There are some differences in the clinical features between single and bipolar depression. The biological marker of bipolar depression is serum FT4.
