摘 要:目的探讨正常人吞咽液体食团及固体食团时的食管动力学差异。方法纳入2016年1-12月40名吞咽正常的志愿者进行高分辨食管测压,比较吞咽液体食团及固体食团时,食管动力学参数的差异。结果 (1)食管上括约肌动力学差异:吞咽液体食团时,松弛后残余压大于吞咽固体食团(P=0.000),松弛持续时间差异无统计学意义(P>0.05)。(2)食管体部动力学差异:吞咽液体食团时,食管全段蠕动波幅度显著低于吞咽固体食团(P=0.003);食管全段蠕动波传递速度快于吞咽固体食团(P=0.000);食管近段蠕动波持续时间慢于固体吞咽(P=0.002)。(3)下食管括约肌动力学差异:吞咽液体食团时松弛后残余压、松弛率以及松弛持续时间均大于吞咽固体食团(P<0.05)。结论两种食团吞咽在食管动力学参数上有差异有统计学意义,在临床中可为单独设立吞咽固体食团时的参考值提供依据。Objective To evaluate the difference of esophageal motility between normal swallowing liquid food group and solid food group.Methods A total of 40 normal volunteers were included in the high resolution esophageal manometry in 1-12 months of 2016 to compare the differences in esophageal motility between swallowing liquid and solid food groups.Results(1)Differences in motility of upper esophageal sphincter:the integrated relaxation pressure after swallowing liquid was greater than that of the swallowing solid mass(P=0.000),and there were no statistical significance of the relaxation duration(P〉0.05).(2)Differences in motility of the esophageal body:When swallowing liquid food group,the total esophageal peristaltic amplitude was significantly lower than that of swallowing solid food group(P=0.003).The total peristaltic wave of the esophagus was faster than the swallowing solid food group(P=0.000).The peristaltic wave duration of the esophagus was slower than that of solid swallowing(P=0.002).(3)Differences in motility of lower esophageal sphincter:the integrated relaxation pressure,relaxation rate and laxity duration of swallowing liquid were greater than that of swallowing solid food group(P〈0.05).Conclusion There is a significant difference between the two groups in the parameters of esophageal motility,and the reference value can be provided in the clinical setting.
