摘 要:目的观察牛痘疫苗致炎兔皮提取物注射液联合甲钴胺防治含奥沙利铂方案化疗所致的消化道肿瘤患者神经毒性的效果。方法将采用奥沙利铂联合卡培他滨方案化疗的78例消化道肿瘤患者随机均分为治疗组和对照组,治疗组39例患者化疗期间给予甲钴胺片和牛痘疫苗致炎兔皮提取物注射液,对照组39例患者仅给予甲钴胺片。比较观察2组患者化疗完成情况、神经毒性的发生情况、体力状况ECOG评分。结果完成6周期化疗病例治疗组为31例(83.8%),对照组为24例(64.8%),治疗组高于对照组,但差异无统计学意义(P>0.05)。治疗组急性神经毒性发生率为76.2%,对照组为77.9%,差异无统计学意义(P>0.05)。慢性神经毒性方面,治疗组和对照组患者在治疗2周期后各级发生率比较差异无统计学意义(P>0.05),4周期、6周期后的神经毒性的1级,2、3级发生率方面比较差异均有统计学意义(P均<0.05)。2组体力状况ECOG评分在治疗前,治疗2、4、6周期比较差异均无统计学意义(P均>0.05)。结论牛痘疫苗致炎兔皮提取物联合甲钴胺可显著减轻消化道肿瘤患者含奥沙利铂方案化疗所致的慢性神经毒性,疗效确切,值得临床进一步探索。Objective To observe the curative effect of analgecine combined with mecobalamin on neurotoxicity induced by chemotherapy with oxaliplatin in the treatment of gastrointestinal cancer. Methods Seventy-eight patients with gastrointestinal cancer treated with oxaliplatin combined with capecitabine were randomly divided into the treatment group and the control group. The 39 patients in the treatment group were given with mecobalamin tablets and analgecine,and 39 patients in the control group received only mecobalamin tablets. The completion of chemotherapy,neurotoxic reactions and ECOG score were compared in the two groups. Results There were 31 patients(83. 8%) in the treatment group and 24 patients(64. 8%) in the control group who had completed six cycles of chemotherapy,the treatment group was superior to the treatment group,but the difference was not statistically significant( P〉0.05). The incidence of acute neurotoxicity was 76. 2% in the treatment group,and was 77. 9% in the control group,the difference was not statistically significant( P〉0.05). In the chronic neurotoxicity,there was no significant difference in the incidence of all levels of neurotoxicity between the treatment group and the control group after two cycles of chemotherapy( P〉0.05). The neurotoxic reaction in level one and two or three on the incidence showed a significant difference after four and six cycles of chemotherapy( P〈0.05). There were no statistically significant differences in the ECOG score between the two groups before and after 2 cycles,4 cycles and 6 cycles of chemotherapy. Conclusion Analgecine combined with mecobalamin could significantly relieve neurotoxicity induced by chemotherapy with oxaliplatin in patients with gastrointestinal cancer,which was proved effect and worthy of further clinical exploration.
