观察脾脏硬度与肝硬化门静脉高压参数的相关性

摘　　要：目的探究脾脏硬度与肝硬化门静脉高压参数的相关性。方法将2016年6月—2017年6月在该院接受治疗的35例肝硬化代偿期患者作为观察组,并将同期该院35例慢性乙型肝炎患者作为对照组,对两组患者肝脏及脾脏硬度予以瞬时弹性成像仪(Fibro Touch)检测,门静脉主干内径及脾脏厚度予以超声检查,肝硬化食管胃底静脉曲张度予以电子胃镜观察。结果肝硬化患者肝硬度值与脾硬度值呈现出明显的正相关(r=0.802,P=0.012);脾脏硬度值与肝硬化门静脉高压相关参数密切相关(P<0.05);中-重度食管胃底静脉曲张患者脾硬度及肝硬度均显著高于无-轻度患者,差异有统计学意义(P<0.05);经过Spearman相关性分析,显示食管胃底静脉曲张程度与脾硬度值呈现出明显的正相关(r=0.631,P=0.023)。结论脾脏硬度与肝硬化门静脉内径、脾脏厚度以及食管胃底静脉曲张密切相关,可将脾脏硬度作为肝硬化门静脉高压的重要指标。Objective This paper tries to explore the correlation between the spleen hardness and the parameters of portal hypertension in liver cirrhosis. Methods 35 patients with liver cirrhosis who were treated in the hospital from June 2016 to June 2017 were considered as the observation group. In the same period, 35 patients with chronic hepati- tis B in the hospital were used as a control group. The spleen hardness was measured with a FibroTouch （FibroTouch） instrument. The diameter of the main trunk of the portal vein and the thickness of the spleen were examined by ultra- sound. The degree of esophageal and gastric varices of the liver cirrhosis was observed by electronic gastroscopy. Re- suits There was a significant positive correlation between liver stiffness and spleen stiffness in patients with liver cir- rhosis（r=0.802,P=0.012）; spleen hardness was closely related to related parameters of portal hypertension in liver cirrho- sis （P〈0.05）; moderate-to-severe esophageal fundus, spleen stiffness and liver stiffness in patients with varices were significantly higher than those in non-mild patients （P〈0.05）, with statistically significant, Spearman＇s correlation anal- ysis showed that the degree of esophageal varices and spleen hardness values were significantly positive correlation （r= 0.631, P=0.023）. Conclusion Spleen stiffness is closely related to the diameter of portal vein of liver cirrhosis, spleen thickness and esophageal varices, and spleen hardness can be used as an important indicator of portal hypertension in liver cirrhosis.
