肺癌患者治疗前血清肿瘤标志物水平与肿瘤骨转移之间的关系分析

摘　　要：目的分析肺癌患者治疗前血清五种肿瘤标志物水平与肿瘤骨转移之间的关系。方法选择2014年1月至2017年6月在我院就诊的肺癌患者90例作为研究对象。根据临床、影像学、病理等确诊为骨转移患者作为骨转移组,未发生骨转移患者作为未转移组。本次研究共确诊肿瘤骨转移38例。根据患者转移病灶将骨转移情况分为3级:Ⅰ级、Ⅱ级、Ⅲ级。采集患者清晨空腹外周血5m L,采用德国罗氏公司的ELECYS 2010型全自动电化学发光免疫分析测定仪,检测CA125、CEA、CA153 CYFRA21-1、NSE等肿瘤标志物。结果骨转移组患者CA125、CEA、CA153、CYFRA21-1、NSE水平显著高于未转移组(P<0.05),骨转移分级越高患者CA125、CEA、CA153、CYFRA21-1、NSE等肿瘤标志物水平越高(P<0.05),CA125特异度较高,但是灵敏度较低,CEA特异度及灵敏度均>60,CA153、NSE特异度及灵敏度均>70,但CA125、CA153、CYFRA21-1水平与肿瘤骨转移之间无显著相关性。结论 CA125、CEA、CA153、CYFRA21-1、NSE五种肿瘤标志物水平均随着骨转移分级增加而增加,但是CA125、CA153、CYFRA21-1与肿瘤骨转移之间无显著相关性,CEA、NSE与肿瘤骨转移显著相关。Objective To analyze the relationship between serum tumor markers and tumor bone metastasis before treatment in lung cancer patients. Methods 90 patients with lung cancer from January 2014 to June 2017 were selected as the subjects. According to clinical imaging and pathology,patients with bone metastases were taken as the bone metastases group,and patients without bone metastases as the non-metastatic group. In this study,38 cases of tumor bone metastasis were diagnosed. According to metastatic lesions,they were divided into three levels of bone metastases： Ⅰ,Ⅱ and Ⅲ level. 5 m L fasting peripheral blood were collected in the morning,and analyzed by German ELECYS 2010 type automatic electrochemical luminescence immunoassay. The levels of CA125,CEA,CA153 CYFRA21-1,NSE and other tumor markers were detected. Results The levels of CA125,CEA,CA153,CYFRA21-1 and NSE in the metastasis group were significantly higher than those in the non-metastasis group（ P〈0. 05）. The levels of CA125,CEA,CA153,CYFRA21-1,NSE and other tumor markers increased with bone metastasis increase（ P〈0. 05）. The specificity of CA125 was higher,but its sensitivity was lower. The specificity and sensitivity of CEA was higher than 60,and the specificity and sensitivity of CA153 and NSE was higher than 70,but the levels of CA125,CA153 and CYFRA21-1 had no correlation with bone metastasis. Conclusion The levels of CA125,CEA,CA153,CYFRA21-1 and NSE increase with the increase of bone metastasis grade,but there is no significant correlation between the levels of CA125,CA153 and CYFRA21-1 and tumor bone metastasis. CEA and NSE are significantly associated with tumor bone metastasis.
